Job Description JOB SUMMARY Under the general supervision of the Program Director or Designee, functions as part of a team providing client-centered, strength-based rehabilitation and recovery services to assigned consumers that relate to the care, nurturing, treatment, education, socialization, habilitation, recovery/healing and support of adults diagnosed with a mental illness and in some instances, co-occurring substance abuse disorder. Position: Outreach Worker Reports To: Assistant Program Manager Location: 444 Thomas Boyland Street, Brooklyn, NY 11212 What The Outreach Worker Does: • Reach out and market the program to the community via street outreach in order to recruit clients., • Assist in organizing responses to all shootings that occur in the targeted catchment area within 72 hours of receipt of notice of the shooting or killing., • Prepare marketing materials and market the program to community., • Attend and help facilitate social/peer support events (i.e., community events and shooting response meetings, etc)., • Review all documentation establishing clients' eligibility for program and make file copies., • Assist clients in completing all CAMBA intake applications and forms., • Create and maintain client files., • Conduct initial intake or assessment of clients and clients' families and/or periodic reassessments., • Follow-up with clients and with referral organizations regarding client contact and progress with referral organization., • Work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency., • Recommend and implement strategies to persuade clients to participate more fully in this process., • Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, etc., • Monitor clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via progress notes., • Recommend closing of cases in which clients have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area)., • Provide all required information for weekly/monthly/quarterly/annual reports., • May conduct client’s and/or families’ periodic re-assessments., • May schedule appointments for client with referral organizations., • May escort clients to appointments (educational, medical, social service, etc.)., • Equally at home conversing with gang members, community activists, law enforcement personnel, and public officials., • Subject to a criminal background check and random drug testing., • No pending criminal cases or prior convictions for sexual assault, child abuse or domestic violence or history thereof., • Strong knowledge of the Brownsville community., • Familiarity with Brownsville 73rd Police Precinct community challenges of teens and young adults there; strongly preferred., • Ability to work a flexible schedule including morning, evening and weekends as needed., • Must have understanding of, sensitivity and commitment to working with families of youth that have experienced gun related trauma., • Case Managers should be prepared for various outdoor weather conditions during their regular workday. Job Description Summary: Streetwork Project Lower East Side is an innovative program for homeless youth providing drop-in, crisis shelter, and outreach, utilizing a harm reduction approach. Under this umbrella, Senior Case Managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale, and survival sex). Services provided by Senior Case Manager include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Senior Case Manager also works alongside program leadership in providing input towards program planning to meet the mental health and substance use harm reduction needs around staff development and client services. Senior Case Manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Intake and assessment for individuals seeking assistance, • Triage counseling, • Mediation for internal conflicts, • Engagement of adolescents into program structure (harm reduction philosophy, rules, mutual respect), • Support Drop-in staff in providing youth access to daily living needs (food, clothes, hygiene supplies, space to rest), • Provide supportive and crisis intervention counseling utilizing a trauma-based intervention (ARC) for youth with more complex mental health needs and challenges., • Provide trainings on overdose prevention, Narcan, as well as provide syringe exchange and other harm reduction supplies with clients, • Collaborate and coordinate with on-site mental health providers to ensure clients access to care., • Work in collaboration with therapist and Supervising Social Worker (SSW) to help develop treatment plans for youth with special needs experiencing severe mental health challenges, • Provide assistance to SSW in facilitation of clinical group process and debriefs, • Collaborate with SSW in identifying trainings around staff development, • Provide support to Supervising Coordinators and Coordinators in absence of directors around scheduling and oversight of the space., • Serve as an advocate and liaison for clients negotiating the shelter system and other social service programs, • Enrollment of clients for Public Assistance & Food-stamps, • Coordinate with social worker in assessing clients’ eligibility and assist with processing for SSI benefits, • Assist youth in application for Supportive Housing and SSI., • Complete psychosocial reports on youth for Supportive Housing & SOAR screening tools for SSI, • Advocacy on behalf of under 18-year-old clients for Administration for Children’s Services support, • Facilitate self-care and coping and risk reduction support groups, • Provide group led facilitation and conducting of educational presentations on HIV/AIDS, risk reduction, and other topics related to sub-populations of homeless youth, • Conduct outreach to homeless and street involved young people, as assigned, • Assist in community outreach and trainings on harm reduction techniques, including Narcan training; overdose prevention; fentanyl testing, • Escort homeless youth to appropriate services and provide transportation as part of case plan, • Preparation of case records, prepare monthly and quarterly statistical reports, • Maintain routine administrative responsibilities to include the preparation of correspondence, maintaining files, mail, email, and telephone., • Participate at staff trainings, staff meetings and staff group process workshops & case conferencing, • Participate in weekly supervision.
