Our outreach team provides mobile case management services to a caseload of over 400 chronically street homeless adults, with the ultimate goal of ending their homelessness. The Clinical Coordinator directly supervises the team’s case management staff and works with the Program Director to administer and sustain the program. BronxWorks operates three 24-hour programs that include a drop-in center, a 50-bed safe haven for chronically street homeless adults, and our borough wide mobile street outreach program. Position Overview As an LMSW providing Early Intervention services, you will support children from birth to age 3 with developmental delays. Services are delivered in home and community environments in partnership with families and other care team members. You will use your clinical expertise to provide both direct services and family support.
