Growing together. \n \n Position Highlights \n In Optum’s Senior Living Program, people are our priority - starting with our dedicated clinicians. We believe in supporting you both personally and professionally to help you reach your full potential. Our care model focuses on assisted living and other community-based settings, blending palliative care with internal medicine. The Nurse Practitioners and Physician Assistants within Senior Living provide care to our highest-risk patients and are part of an interdisciplinary team. \n \n We offer extensive training in our unique approach, and because we are not fee-for-service, our focus is on quality patient outcomes, not volume. With a manageable expectation of approximately 4-6 patient visits per day, you’ll have the time to invest in your patients’ well-being and build meaningful relationships. \n \n Primary Responsibilities: \n\n * Conduct comprehensive assessments \n • Manage patients through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions \n • Manage patients during a short term sub-acute rehabilitation episode \n • Ensure accurate and complete documentation, including ICD 10 conditions \n • Communicate and collaborate with the interdisciplinary care team \n • Conduct advanced illness and advanced care planning conversations \n • Provide patients and caregivers with counseling and education \n • Perform comprehensive assessments and document findings in a concise/comprehensive manner that is compliant with documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations \n • Facilitate agreement and implementation of the patient’s plan of care by engaging the facility staff, families/responsible parties, primary and specialty care clinicians \n • Coordinate care as patients transition through different levels of care and care settings \n • Review orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the patient’s needs and wishes \n • Function independently and responsibly with minimal need for supervision \n\n \n What Makes an Optum Career different? \n\n • As the largest employer of Nurse Practitioners and Physician Assistants, we have a best-in-class employee experience and enable you to practice at the top of your license \n • We believe that better care for clinicians equates to better care for patients \n • We are influencing change collectively on a national scale while still maintaining the culture and community of our local care organizations \n • We grow talent from within. Required Qualifications: \n\n • For Nurse Practitioners:\n\n, • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program\n, • Active and unrestricted Nurse Practitioner license in the state which you reside, or ability to obtain by start date \n, • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:\n\n, • Family Nurse Practitioner\n, • Adult Nurse Practitioner\n, • Gerontology Nurse Practitioner\n, • Adult-Gerontology Acute Care Nurse Practitioner\n, • Adult-Gerontology Primary Care Nurse Practitioner\n\n\n\n\n, • For Physician Assistants:\n\n, • Graduate of an accredited Physician Assistant degree program\n, • Active and unrestricted Physician Assistant license in the state which you reside, or ability to obtain by start date\n, • Board certified by the National Commission on Certification of Physician Assistants (NCCPA)\n\n\n, • Active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice) (SCC04)\n, • Ability to gain a collaborative practice agreement, if applicable in your state \n, • Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment \n, • Access to reliable transportation\n\n \n Preferred Qualifications: \n\n * New graduates are encouraged to apply\n * 1+ years of clinical experience in practice in senior living, geriatric medicine, or home care \n • Experience in meeting the healthcare needs of patients with complex behavioral, social and/or functional needs \n • Understanding of Geriatrics and Chronic Illness \n • Understanding of Advanced Illness and end of life discussions \n • Proficient computer skills including the ability to document medical information with written and electronic medical records\n\n \n PLEASE NOTE Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease.
