Job Description Dialysis Chronic Care Registered Nurse Title: Dialysis Chronic Care Registered Nurse (RN) 12 months or more of Recent Dialysis experience required PURPOSE AND SCOPE: This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients.
