Job DescriptionDescription: Signing Bonus $5,000! The NFP Nurse Home Visitor (NHV) is responsible for providing comprehensive nursing services to first-time parent(s) eligible for the NFP program. Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice, case management and care coordination, adherence to the NFP model, and to policies, procedures, guidelines, and standards of NFP and of PMCH. This role is central to promoting healthy pregnancies, positive parenting practices, and early childhood development. Working within a strength-based, culturally responsive framework, this position supports improved health outcomes and family well-being in underserved communities. Essential Functions • Provide in-person home visitation to eligible families and carry a caseload of up to 25 clients in accordance with the NFP Model Elements and guidelines, • Complete all required NFP education and nurse-home visitor competency requirements, including PIPE: Partners in Parenting Education, and ongoing professional development and webinar training, • Develop therapeutic relationships and utilize concepts of reflection and motivational interviewing with families in a home visiting environment, • Follow nursing process in implementing NFP into nursing practice, • Assess the physical, emotional, social, and environmental needs of parents and their families as they relate to the NFP domains, • Assist parents and their families in establishing goals and outcomes; provide education, support, and referral resources in attaining goals; evaluates clients’ progress and plans home visits in accordance with clients’ goals and NFP outcomes, • Consult and collaborate with other professionals involved in providing services to families, • Meet with NFP Nurse Supervisor weekly for clinical supervision and schedules joint home visits every four months. Requirements: • Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred., • Current New Jersey RN license (in good standing) required., • BLS certification required., • Two years' experience in maternal/child health, public health, home visiting, or mental/behavioral nursing preferred, • Strong documentation skills to ensure data entry is completed correctly on time, • Bilingual English/Spanish required, • Excellent verbal, written, and interpersonal communication skills, • Ability to work independently and manage client visit schedules to meet program visit requirements, • Valid driver’s license and reliable transportation required for regional travel PMCH aims to foster a workplace culture that prioritizes diversity, equity, inclusion, and belonging.
