Provide case management services to young adults and adults with serious and persistent mental health diagnosis in a day treatment intensive program. Support client engagement, recovery-oriented care planning, service coordination, crisis intervention, and documentation under clinical supervision. Ensure services are person-centered, trauma-informed and culturally responsive. Position Details: • Full-Time, • Locations: Palmdale Key Responsibilities • Conduct intake assessments, strengths, and needs evaluations, and ongoing reassessments for assigned case load., • Develop, implement, and monitor individualized recovery-oriented service plans in collaboration with clients and multidisciplinary team., • Provide direct support including individual case management, brief counseling, psychoeducation, and group facilitation as needed, • Coordinate referrals and linkages to community resources (housing, employment, benefits, healthcare, substance use services), • Monitor client progress, adherence to treatment goals, and adjust plans, participate in treatment team meetings, and care conferences., • Provide crisis intervention and de-escalation, follow program safety and emergency procedures, coordinate with crisis/ER services when necessary., • Maintain timely, accurate clinical documentation in electronic health records (intakes, progress notes, service plans, incident reports) according to agency and regulatory standards, • Advocate for clients' needs with external providers, payers, and community agencies., • Ensure compliance with confidentiality (HIPAA), ethical standard, and program policies, • Participate in staff trainings, supervision and quality improvement initiatives. Qualifications • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or related field., • Minimum 1 year experience working with adults with mental health diagnosis (case management, community mental health, residential or day treatment experience preferred)., • Strong written and verbal communication skills, competency in documentation, and basic computer skills (EHR, MS Office), • Ability to manage a caseload, prioritize tasks, and work both independently and as part of a multidisciplinary team., • Valid driver's license and reliable transportation (If role includes community outreach/transportation), • Experience in recovery -oriented, trauma-informed care and crisis intervention, • Familiarity with benefits systems (SSI, SSDi, Medicaid) and vocational/housing supports, • Bilingual skills (Spanish) a plus Benefits Help Group offers wonderful training opportunities, a supportive, professional work environment, and great benefits, including: • Insurance plans for Medical, Dental, Vision, and Life Insurance - options for 100% employer sponsored plans, • 403b Retirement Plan, • 529 College Savings Plan, • Employee Assistance Program, • Flexible Spending Account, • Vacation and Sick Leave, • Benefit Hub - Employee Rewards and Incentives, • Pet Insurance The salary range represents the low and high end of the salary someone in this role may earn as an employee of Help Group. Salaries will vary based on various factors, including but not limited to, professional and academic experience, certification, training, responsibilities of the position, and other business and organizational needs. The range listed is just one component of our total compensation package for employees. Salary decisions are dependent on the circumstances of each hire.
