The Medspan program provides patient education and care coordination around HIV, hepatitis C, and associated conditions such as hepatitis B and sexually transmitted infections (STIs). Medspan team members support patients through medication adherence support, discharge planning, and connection to care services. The Medical Director works across the Correctional Health Service Medicine service, with MedSpan staff and with other interdisciplinary Correctional Health Services (CHS) staff in all facilities to provide, improve, and evaluate patient centered infectious disease care. Responsibilities include: (Detailed Task) Clinical Responsibilities: • Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools., • Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes., • Deliver clinical consultation and guidance related to infectious diseases to site medical directors (SMD), infection control teams, and clinical staff. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Nurse Practitioner will be matched with the population that most closely meets their clinical expertise, desired work location and schedule, and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs, • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization, and escalate care when clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care., • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Conduct routine safety rounds in all facilities where CHS employees are located while identifying potential environmental safety hazards to the Department of Corrections (DOC) for mitigation. The Occupational Safety & Environmental Health Coordinator will engage with staff to identify safety gaps or environmental health concerns, provide on-going support and follow-up for staff involved in workplace violence incidents and/or who have suffered occupational injuries. This position serves as a central resource for safety education, incident response, and environmental risk mitigation across all CHS sites and services. Correctional Health Services The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs., • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care., • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner., • Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients., • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication., • Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed., • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics., • Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist, • Other duties as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director or Chief of Service for Psychiatry, or their respective designees. Position Overview Under the supervision of the Site Medical Director, Nurse Practitioners will deliver comprehensive, compassionate, and thoughtful care to patients with complex chronic diseases within the New York City jail system. This role involves collaboration with physician leadership to provide general primary care, which includes conducting detailed histories and physical exams, diagnosing and managing both acute and chronic illnesses, and assessing the necessity for consultative services. In addition, the Nurse Practitioner will engage as part of an interdisciplinary team, focusing on patients with chronic medical conditions or physical disabilities who do not require acute inpatient admission. This approach ensures a holistic view of patient care and leverages the expertise of diverse healthcare professionals to optimize treatment outcomes. Responsibilities include: (Detailed Task) • Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated., • Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable., • Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits., • Implement plans for patient care utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care., • Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate., • Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented., • Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed., • Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags)., • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care., • Ensure that all progress notes and orders are signed before the end of the shift., • Respond to emergencies in a timely and professional manner., • Notify the staff MD and Urgicare about 3 hour runs and EMS activation., • Complete special housing rounds when assigned., • Be familiar with quality of care and population health indicators. The Supervising Psychiatrist is a psychiatric leader of a facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Senior Psychiatrist (if applicable) to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff. Detailed Tasks: • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This 109-unit residence will provide housing for 39 individuals referred by the NYC Human Resources Administration (HRA), and the remaining 70 units will be apartments for moderate-income households. The Social Service Supervisor will provide clinical and administrative supervision to members of the Clinical team. The successful candidate will have strong clinical and supervisory skills along with experience working with individuals diagnosed with co-occurring disorders and/or trauma, as well as relevant New York City and State agencies and funding sources. Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Nurse Practitioner will be matched with the population that most closely meets their clinical expertise, desired work location and schedule, and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: * Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs * Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information. * Evaluate patients for the need for a higher level of care, including hospitalization, and escalate care when clinically appropriate. POSITION SUMMARY: A Driver- CDL (B) is responsible for safely operating a collection truck, and providing prompt, courteous and complete waste removal services for customers. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent, affordable, and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs. We are seeking a Case Manager for The Doe Fund's newest permanent supportive housing for homeless individuals living with a serious mental illness and/or history of substance use. The housing program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 109-unit residence will provide housing for 39 individuals referred by the NYC Human Resources Administration (HRA), and the remaining 70 units will be apartments for moderate-income households. The Case Manager will play a crucial role in supporting the residents referred by HRA. Responsibilities include providing comprehensive case management services and counseling tailored to the needs of these individuals. The successful candidate for this position should demonstrate strong clinical skills and prior experience working with individuals diagnosed with co-occurring disorders and/or trauma. RESPONSIBILITIES: • Maintain a caseload of individuals diagnosed with serious mental illness and/or substance use disorder and provide supportive case management services to them, • Develop and facilitate therapeutic groups for residents, • Maintain resident records in compliance with agency and funding standards, • Conduct and review assessments, • Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency, • Monitor and ensure residents are linked to appropriate treatment and services, • Provide counseling and crisis intervention as necessary, • Assist residents in securing and maintaining benefits, • Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs, • Monitor and assist residents with rent payments and budgeting, • Conduct regular unit inspections, • Educate residents about approaches to mental health and substance use, • Compile resources on mental health information and treatment strategies, • Attend ongoing training to enhance the services provided to the residents, • Assist with the completion of reports as needed, • Other duties as assigned QUALIFICATIONS: • A Bachelor’s degree in Psychology, Mental Health Counseling, Social Work, or a relevant field. Master’s degree preferred., • At least 3 years of experience working with homeless individuals diagnosed with serious mental illness and/or substance abuse disorder and/or in trauma settings, • Expertise in Evidence-Based Practice, including Critical Time Intervention, Motivational Interviewing, and Harm Reduction techniques, • Excellent clinical and case management skills, including knowledge of benefits in the NYC Metro area, • Sensitivity to community issues and needs, • Outstanding interpersonal and communication skills, both oral and written, • Strong ethical standards, confidentiality, and personal integrity, • Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint) Interested? About the Role As our Advertising Campaign Operations Associate, you will support our digital advertising campaign execution and optimization while serving as a key point of contact for cross-functional teams.
