This includes providing case management support to address rental and utilities arrears, coordinating home modification and remediation needs, and connecting clients to appropriate social care resources that promote housing stability and overall well-being. Key responsibilities include conducting housing retention intakes and assessments; developing individualized housing stabilization plans; providing and coordinating direct services in alignment with those plans; and maximizing housing tenure. The Housing Advocate must be knowledgeable about the full range of housing options available to individuals with substance use or co-occurring disorders. The role also requires building and maintaining strong relationships with housing providers, prospective landlords, brokers, agents, and property management companies. In addition, the Housing Advocate will collaborate with community partners to develop and maintain a comprehensive housing resource manual with services available on Staten Island and throughout the other boroughs. DUTIES & RESPONSIBILITIES: • Conduct initial and ongoing housing needs assessments with referred clients, members of the Next Step Resource and Recovery Center, Peer Training Institute, and their families., • Initiate and complete the process to secure decent, affordable permanent housing, including preparation and submission of the 2010E housing application., • Serve as the Social Care Referral Coordinator, receiving and managing referrals for Housing and Tenancy Services through the CHASI Social Care Network, including case management for rental and utility arrears, home modification, remediation, and other tenancy supports., • Assist clients in building the skills and supports needed to maintain their households and finances, perform activities of daily living independently, uphold the terms of their leases, and prevent avoidable evictions., • Intervene and provide crisis mitigation strategies to maximize tenant safety and security., • Assess and promote ongoing linkage to medical, behavioral health, and community support services, including intensive case management for those with multiple needs. Communicate and coordinate housing-related activities with referring program staff to ensure continuity of care and service connection., • Research, identify, and catalog current housing options locally and across boroughs for individuals in recovery, including those with specialized needs such as mental health support, family housing, or safety-related accommodations., • Develop and maintain a Housing Readiness Assessment and individualized housing stabilization plans., • Establish and maintain relationships with housing providers, landlords, brokers, agents, and property management companies to expand housing opportunities., • Communicate, engage, and advocate on behalf of clients to support housing stability and self-sufficiency., • Prepare timely, accurate weekly and monthly data for program and funder reporting requirements., • Participate actively in team meetings, supervision, and case conferences., • Collaborate with community partners to contribute to a comprehensive housing resource manual for Staten Island and the surrounding boroughs., • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values., • Other duties as assigned. Job Description Status: Full-time, Mon-Fri. 9 am - 5 pm (hours may vary) FLSA Classification: Non-Exempt Job Location: Upper West Side, New York Reports to: Program Director/Assistant Program Director Hourly range: $27.47 - $32.96 We are seeking a dynamic and engaging Case Manager who has experience working with formerly homeless, mentally ill single adults in a supportive housing environment. The Case Manager will be expected to assist this population in stabilizing their lives, maintaining their housing stability, and attaining their physical, emotional, vocational, educational, and social goals. The ideal candidate will have solid clinical knowledge as well as experience working with an individual that has been impacted by mental illness, substance abuse, legal, criminal justice, and medical issues. Additionally, the Case Manager will be passionate about helping people; be a proactive and creative problem solver; and work well both as a part of an interdisciplinary team and independently. Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions. • Provide integrated counseling and case management services that effectively engage residents and promote health and wellness, personal growth, and housing stability., • Collaborate with residents in the development of client-centered, recovery-oriented service plans that identify short and long-term goals., • Utilize a harm reduction approach to support a diverse caseload of formerly homeless individuals around issues of mental health and substance use., • Make referrals and serve as a liaison with various care coordination agencies, health, and mental health providers, substance use programs, and employment providers to secure services., • Actively engage residents through regular apartment visits, community outings, and escorts to appointments as needed., • Develop safety plans for residents and work with the team to de-escalate clients and collaboratively manage crises as they arise., • Assist in the development and implementation of groups and programs that are appropriate for the resident's needs and foster an inclusive, respectful community., • Develop and maintain confidential records for each resident according to the mandated schedule documenting all relevant interventions and assessments as well as correspondence with collateral services., • Collaborate with the Leasing and Compliance team to ensure housing stability., • Participate in staff and clinical meetings and individual supervision to ensure best practices in the performance of role responsibilities., • Participate in a robust training program that includes training in evidenced-based models to ensure the highest level of service is provided to clients., • Perform other job-related functions, as assigned. Required Education and Experience: • BA/BS from an accredited school is required, preferably in a related field, such as social work, sociology or psychology; Master's degree from an accredited school is preferred., • Must have a valid Food Handler’s License, or the willingness and ability to obtain one within six (6) months of hire, is required., • One year of experience working with single adults impacted by poverty, chronic homelessness, mental illness, substance abuse, and/or medical issues is required., • Ability to work independently as well as a member of an interdisciplinary team., • Highly organized and able to manage a diverse caseload of clients with diverse needs., • Able to work well under pressure including in crisis situations and able to be flexible with work schedule when necessary., • Track record of proactive, strength-based work with individuals and families., • Experience with ETO, or other electronic case management systems., • Experience working with evidence-based models, specifically Motivational Interviewing, is preferred., • Preferred: Knowledge of a mental health, substance using, or chronically homeless population.
