Bridging Care is a Care Coordination Organization (CCO) within Washington State's Health Homes program. We serve Medicaid members with serious chronic conditions who have nowhere else to turn — connecting them with a dedicated Care Coordinator who builds a health plan and stays in their corner. As a Health Home Associate, your job is to be the first contact: you reach members, enroll them in the program, and build the documented service history and training foundation that qualifies you for the Care Coordinator role — typically within four to six months. How the pathway actually works: To become a Health Home Care Coordinator in Washington State, you need to qualify under a state-defined education and experience tier — or qualify through a formal exception process that Bridging Care files on your behalf. This role is specifically designed to build that case. After 4–6 months of documented performance, Bridging Care supports your Health Home CC Exception (waiver) application, including the supervision plan and documentation the state requires.
