Job DescriptionKitchen Food Service Specialist Monday to Friday 7:00am to 3:30pm Community Psychiatric Institute, a leader in Behavioral Healthcare and Substance Abuse treatment in Essex County for over 50 years has an opening for a full-time Cook - Kitchen Food Service Specialist in our adult Partial Care Program. The Children and Youth Specialist is responsible for the following tasks: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients. Students who attend sessions with our learning specialists generally require support in math skills, foundational literacy skills, reading comprehension, and/or written expression. The learning specialist most often provides intervention in one-to-one sessions or in a dyad. The child-centered approach at Quad Prep is highly collaborative, a trait that the learning specialist must uphold to provide personalized support to each student. While most students who work with our learning specialists are performing below grade-level, learning specialists do, on occasion, work with students who are far exceeding grade-level expectations. Job Description The Data Specialist is responsible for implementing and maintaining compliance with all federal evaluation requirements (SUPRT A/C) of a five-year collaborative initiative (currently in Year 3), funded by the Substance Abuse and Mental Health Services Administration (SAMHSA) National Child Traumatic Stress Initiative (NCTSI) called Project REACH. The goal of REACH is to raise the standard of care and increase access for school aged children experiencing trauma to evidence-based trauma intervention within Safe Horizon’s Child Trauma cluster of programs. These programs include the Safe Horizon Counseling Center (SHCC), the Child Trauma Response Team (CTRT), Safe Way Forward (SWF), and our 5 Child Advocacy Centers (CACs). This child trauma initiative also includes tracking client outcomes to influence citywide systems & practice enhancements in collaboration with national evidence-based treatment model developers, government partners, and a diverse array of internal stakeholders. The Data Specialist takes a lead role in contacting all new REACH clients to conduct baseline, follow-up, and discharge assessments that will allow the program to assess its effectiveness in achieving desired outcomes for traumatized children and their families. The Data Specialist is required to maintain meticulous data tracking to ensure full compliance with all federal reporting requirements and regulations. The availability of new clients to participate in these outcome evaluation activities will vary. Job Description The STAR Case Manager will work in partnership with Peer Recovery Specialist (PRS) to provide support for individuals in Union county that are facing challenges that are associated with Opioid Use Disorder, such are such as homelessness, incarceration, legal matters, employment, education, transportation, social services, healthcare, involvement with child welfare, childcare, health insurance, and linkage to treatment. POSITION RESPONSIBILITIES: • Provide a comprehensive case management assessment that addresses life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family strengths/needs;, • Develop an Integrated Family Case Plan that is person-centered and includes strategies for recovery. The plan shall identify priorities, desired outcomes and the STAR - 8 strategies and resources to be used in obtaining outcomes based on the case management assessment;, • Link to appropriate care and resources in the community including resources that address specialized needs, such as agencies providing services related to HIV/AIDS, mental health disorders, chronic and acute health problems and problems stemming from involvement with the criminal justice system;, • Promote the STAR individual’s recovery to his/her fullest potential by facilitating assessment of skills, including literacy, educational opportunities (GED, certification programs), job readiness skills, vocational training, employment placement and retention and career development;, • The STAR case managers are required and must document that each individual was offered a visit minimally one (1) hour per week by a staff member, whether that visit was accepted and document their participation in the visit and any services received. These hotlines provide crisis intervention, advocacy, and information and referrals to over 125,000 victims each year. Job Summary: The Case Planner provides comprehensive case management services for all youth who reside in their respective Site i.e. Non-Secure Placement or Limited-Secure Placement facility and their families. The Case Planner should be committed to a service philosophy and objectives that is youth-centered and outcome focused. The position is demanding and highly interactive with the youth and their families, staff and community. Case Planners must be comfortable working with youth and families in the context of the facility, the office and in the community of origin. The Case Planner will maintain a caseload of 15 youth both in the facility and in the community on aftercare. The role of the Case Planner is to streamline services and service plan goals, guide the discharge readiness planning beginning once youth are placed, and follow through with the youth/family in the application of the service plan in the community. Case management responsibilities include all necessary tasks to complete case file reviews, case planning, family engagement, and group service delivery.
