Job Description JOB TITLE: Solution-Based Casework (SBC) Case Planner EMPLOYMENT TYPE: Full-Time LOCATION: Hybrid – 2-3 days in office (Queens Location) REPORTS TO: Solution-Based Casework Supervisor SALARY: $60,356.30 Annually ABOUT AAFSC: For over 30 years, the Arab-American Family Support Center (AAFSC) has proudly served immigrant, refugee, and marginalized communities across New York. JOB OBJECTIVE: To provide high-quality case planning, case work, and case management services to at-risk families referred by the Administration for Children’s Services. These families may be experiencing a wide range of challenges, including personal/marital/relationship problems, financial stressors, acculturation issues, family destabilization, and/or parent/child conflict. The Case Planner provides intensive preventive services to avoid foster care placement and ensure the safety and welfare of children in the home. He or she provides the family with services according to ACS standards and requirements. The Case Planner follows a Solution Based Casework (SBC) practice model. SBC is based on three theoretical foundations: family life cycle theory, relapse prevention/CBT theory, and solution-focused family therapy. Partnership with families in each and every case is critical to reducing risks and solutions should utilize prevention skills to achieve those goals. PRIMARY DUTIES & RESPONSIBILITIES: Case Management & Advocacy * Ensure timely completion of SBC training & certification. * Conduct home visits and office visits to assess child safety and risk elements, complying with ACS requirements. Face-to-face contacts should be conducted twice per month, at a minimum and includes home visits (two home visits must be conducted if children 0-2 years old are present) and office visits (e.g., school, doctor’s office, AAFSC), as well as other out-of-home visits (e.g., park). More visits may be required. * Provide case work/case management services to families receiving Preventive Services (12 cases); which includes but is not limited to providing supportive casework, focusing on service goals, and child welfare laws, improving family relationships and bonding, coping skills, parenting, etc. * Ensure joint-home visits are conducted within 5 business days from the date of referral. * Evaluate and assess the current family functioning including child safety and home safety (e.g., assessing other adults in the home, ensuring safe sleep, working fire alarms, window guards, etc.). * Work in partnership with the family to formulate outcomes & service plans with the family. * Ensure all initial intake documents (AAFSC intake, Domestic Violence screening, child trafficking) are completed and processed accurately using AAFSC internal database (i.e., Apricot). * Advocate for the family by linking them to needed services in the community: medical, mental health, entitlements, day care, home health aide services, substance abuse treatment, anger management, etc. and follow up to monitor participation and progress. * In collaboration with the Conference Facilitator, support the facilitation of Family Team Conferences and Service Termination Conferences, as well as other relevant ACS-led conferences. Family Team Conferences must be conducted within 4 weeks of the FASP due date. * Ensure FASPs are launched 30 days prior to the due date and submitted to Supervisor 3 weeks prior to the due date. * Complete Initial, Comprehensive, all Reassessment FASPs, and Plan Amendments for all Advocate Preventive Only (ADVPO) cases offline. * Complete and Submit Child Trafficking Screening for Child Welfare Services and ADVPO cases. * Evaluate walk-ins and community referrals for needed services and potentially for advocacy cases. * Provide case coverage in the absence of a co-worker (as needed). * Schedule and coordinate monthly visits with FSU workers (applicable to FSU cases only). Quality Assurance, Communication, and Documentation * Complete all documents required by ACS in a timely manner (5 business days) including but not limited to progress notes and FASPs using templates in a manner that reflects the progress or lack thereof, including the reaction of the family to services during the review period or contact. * Complete all ACS and internal forms required for case opening and closing as well as obtain consents and forms needed for other case activities. Obtain and maintain collateral contacts with all other service providers including school and medical reports on a regular basis. * Comply with all data entry requirements for database systems including inputting information in a timely manner (PROMIS, Connections, etc.). * Compile and organize appropriate client intake paperwork, surveys, and forms. Maintain casefiles, database entry, and record keeping in compliance with ACS, OCFS, and AAFSC requirements, in alignment with the Solution Based Casework Model guidelines (as needed). * Appropriately follow incident reporting procedures (as needed). * Maintain relationships with ACS personnel, including DCP workers, joint home visit ACS staff, and FSU workers, for ongoing ACS supervision. * Regularly collaborate and communicate with PPRS staff by responding to emails, maintaining an up-to-date Outlook calendar, participating in supervision meetings, and scheduling office days. * Perform other job-related duties as assigned. Trainings, Meetings, and Audit Preparation * Attend training provided by ACS, Solution Based Casework, and other organizations as required (minimum of 42 hours/6 days each fiscal year) and log training by submitting certificates to the Director of Training and Development. * Attend internal meetings as scheduled, collaborating with the multidisciplinary team (e.g., monthly supervision). * Assist in preparation of internal and external audits (COQI, PAMS, safety alerts, etc.) related to the program operation and delivery of services. With a team of over 900 staff and an annual budget of $130 million, the Center carries out its mission through three core strategies: • Operating Programs that pilot new ideas and address local challenges;, • Conducting original research to evaluate what works—and what doesn’t; and THE OPPORTUNITY The Center for Justice Innovation is seeking a Senior Social Worker to join a team of staff—including social workers and case managers—who work with defendants participating in the Manhattan Felony Alternative to Incarceration (ATI) Court. The ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to the Clinical Director, the Senior Social Worker will be part of a team that provides neutral, third-party assessments and case management services to the ATI Court. The Senior Social Worker will conduct independent clinical assessments, provide referrals to community-based services, offer comprehensive clinical case management, and monitor participant compliance. The Senior Social Worker will be primarily responsible for supporting a caseload of high-risk need program participants and supervising a group of case managers and/or social workers. Additionally, in coordination with the Clinical Director, the Senior Social Worker will help coordinate compliance updates on ATI Court calendar days, help identify gaps in services and forge new partnerships with community-based providers to fill those needs, serve as the team’s training liaison to identify internal training needs and coordinate trainings for staff, and coordinate with the Office Manager regarding participant supplies and other needs. An essential and dynamic feature of the Senior Social Worker’s role will be in close collaboration with key stakeholders. The Senior Social Worker will work regularly with the judge and court staff from New York County Supreme Court who oversee the Felony ATI Court, as well as with other stakeholders, including the Manhattan District Attorney’s Office and New York City defense organizations. Court-based social work is an exciting, unique, and demanding area of practice, and the Senior Social Worker role involves navigating and constructively engaging with differing and, at times, competing stakeholder perspectives. The Wellness and Primary Prevention Specialist will help serve students by providing support in case management, screening and referral and linkage to community resources. The Specialist will support the Wellness Center in providing Passaic County Community College (PCCC) students with any referrals pertaining to mental, emotional, and physical health, as well as overall well-being to on-campus resources and those in the community. The Felony ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to the Clinical Director of Felony ATI, the Social Worker will be part of a team that provides neutral, third-party assessments and case management services to the Felony Alternative to Incarceration (ATI) Court. The Social Worker will be responsible for conducting independent clinical assessments, providing referrals to community-based services, offering comprehensive clinical case management, and monitoring participant compliance. An essential and dynamic feature of the Social Worker’s role will be close collaboration with key stakeholders. The Social Worker will work regularly with the judge and court staff from New York Country Supreme Court who oversee the Felony ATI Court, as well as with other stakeholders, including the Manhattan District Attorney’s Office and New York City defense organizations. Court-based social work is an exciting, unique and demanding area of practice, and the Social Worker role involves navigating and constructively engaging with differing and, at times, competing stakeholder perspectives. Responsibilities include but are not limited to: • Conduct detailed risk-need profiles and clinical assessments for potential Felony ATI Court participants;, • Develop treatment recommendations for services for potential Felony ATI participants to court stakeholders (judge, defense counsel, prosecutor and court staff);, • Provide comprehensive, clinically-informed case management services to a caseload of pre-plea and court-mandated Felony ATI participants to help address issues such as substance use, mental health issues, trauma, domestic violence, housing and vocational needs, and other challenges in a participant’s life (e.g. new arrests, family court cases and other civil legal issues);, • Make appropriate community-based referrals for a variety of social service needs;, • Manage and monitor compliance with treatment mandates, including care coordination with providers;, • Provide written treatment updates for court stakeholders for all court-involved participants;, • Participate in regular interdisciplinary case conferences with court stakeholders;, • Coordinate with court stakeholders on any non-compliance issues and assist clients in getting back into compliance, including re-engaging clients, arranging return on warrants, and implementing graduated responses to encourage future compliance;, • Facilitate and help to develop and/or revise curricula for a range of on-site psychoeducational groups and individual case management sessions;, • Attend court on calendar days, as needed, to help clients navigate court and collaborate with court staff and stakeholders;, • Collaborate and maintain strong relationships with key stakeholders, including the resource coordinator, prosecutors, defense attorneys, and other court and programmatic staff;, • Assist with toxicology testing and reporting, as needed;, • Provide crisis intervention and de-escalation, as needed;, • Provide voluntary referrals to community-based services, as needed;, • Establish and maintain linkages with community partners who can support program work;, • Assist the Clinical Director with the development and implementation of new initiatives; including best clinical practices, and appropriate evidence-based interventions;, • Enter and maintain appropriate up-to-date participant information, including assessment, case management, and case outcome data;, • Provide clinical services as described above to program participants on a voluntary basis post-mandate;, • Participate in clinical group supervision, staff meetings, and trainings, as scheduled;, • Attend evening community-based meetings and events, as needed; and, • LMSW, with a minimum of 4 years in criminal justice and/or a related field is required. SIJC is seeking a Clinician, Supervised Release Program (SRP). Reporting to the Clinical Director, the Clinician, SRP will be licensed a social worker and mental health practitioner responsible for providing therapeutically-informed support to participants. This position will leverage exemplary clinical and interpersonal skills to provide clinical support to participants enrolled in the program. Specifically, the Clinician, SRP will facilitate CBT-informed sessions for all Tier 2 participants, provide short-term counseling for any participant identified as a strong candidate for mental health support, and external referrals to therapy, as appropriate. This position provides hours towards LCSW accreditation. The Clinician, SRP will not carry a standard SRP caseload. Rather, they will work closely with the case management and specialty in-house referral teams to case conference about participants' strengths and needs and coordinate tailored support as appropriate. ACT supports client recovery through a highly individualized approach that provides clients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. The Substance Use Specialist is part of this multidisciplinary team and is responsible for delivering intensive in-home and community-based services to individuals living with severe mental illness, whom have a treatment history characterized by frequent use of psychiatric hospitalization and emergency rooms, involvement with the criminal justice system, alcohol/substance abuse, and lack of engagement in traditional outpatient services. This settlement requires the state to assist residents of impacted adult homes identified as “class members” to move out of adult homes into supported apartments if they choose to and are eligible. They will have the assistance of a care manager to ensure any needed wrap-around services are in place after the move. An assessment determines the care planning that the care manager must implement as part of the preparation for transition out of the Adult Home. In collaboration with the Nursing Assessor and assigned Care Coordinators, the Social Work Assessor will work with other team members to streamline the assessment and placement into supported housing for identified residents of impacted Adult Homes. They will be responsible for making recommendations about the type of housing and the types of services—physical and behavioral health—as well as non-medical services like Meals on Wheels, etc. The Social Work Assessor will complete assessments for Queens Adult Home and SIBN Adult Home Supported Housing Programs. The Social Work Assessor will complete the housing application's psychiatric evaluation and psychosocial portion. The Youth Care Worker AKA Child Care Worker is responsible for ensuring the minors are receiving appropriate nutrition, leisure, and recreational activities. They supervise the minors at all times, maintaining required supervision ratio and ensure all minors are accounted for.
