As a team member of the SPOA unit you would be processing applications for adults with mental illness seeking an array of services, including but not limited to: Assisted Outpatient Treatment, Case management and ACT services, SPA Mental Health Housing, and/or Crisis Respite housing. This requires thorough review of the application and supporting documentation; investigation of documents; case presentations; general knowledge regarding Health Homes and resources in the mental health system; general knowledge of mental health housing in Suffolk County; ensuring court-ordered Mental Health Services are in place and that the person under an AOT order is compliant with those services. In addition to linking individuals to needed services, you will also have a caseload of individuals under an active AOT court mandate, which you will be responsible for receiving, documenting and following up on weekly updates from current providers, as well as requesting and following up on additional documentation as required. This is a very intensive program, where the work can, at times, take on an emergent need. You will need to be organized and able to complete tasks timely and efficiently. It is necessary that you possess good communication and negations skills, and feel comfortable interfacing with agency directors, psychiatrists, hospital social workers, police, and at times attorneys and criminal justice workers.
