As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health. Schedule: Monday- Friday with flexible scheduling, 32 patient contact hours minimum (+ 8hrs of administrative time). Hybrid model of care. Call volume – very low 24/7/365 physician back upRequirements • Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB)) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA)., • Unrestricted DEA licensure and prescribe authority (per state practice regulation)., • You have an active, unrestricted license to practice in NY, or willingness to obtain. Willingness to obtain additional licensures and pursue independent practice if needed based upon business needs., • You have excellent communication and interpersonal skills., • You’re dedicated to staying current with the latest developments in primary care and obesity medicine., • You believe in inclusive, compassionate care., • You believe in shared decision making with a patient. You honor the goals a patient has for their health, and you partner with them to help them achieve those goals., • You have high emotional intelligence and know how to make a patient feel at ease – without judgment and without shame., • You’re willing and excited to try new technologies that are both patient and provider oriented., • You are excited to work with a collaborative team of physicians, APC’s, registered dietitians, medical assistants, health coaches, and other staff., • We are unable to sponsor visas currently, so you are authorized to work in the United States. Responsibilities, • Practice exceptional medicine within knownwell, • Sees adults for primary care, often with a focus on metabolic health, or adults for metabolic health only. Share a panel with MD/DO + other APCs., • Embodies the knownwell mission and values of patient inclusivity in how you deliver care., • Coordinates with the broader care team to ensure the patient has the support they need., • Documents as necessary to ensure accurate billing, including review of remote patient monitoring data., • Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response., • See adults ages 16+ for metabolic health and primary care., • Ensure panel is open to any individual, regardless of their current health status and comorbidities., • Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey. Provide an environment in which patients feel comfortable and free from judgment., • Offer exceptional encouragement as patients reach their goals., • Coordinate across the collaborative care team, • Escalate any concerns to your dedicated site leadership team., • Provide clinical guidance to medical assistants and nursing teams as assigned.
