Description Position Overview: The Life Coach at Lee Goodwin Residence provides comprehensive case management, outreach, and counseling to the residents at our supportive housing facility for young adults aging out of foster care between the ages of 18-28 year olds. The Life Skills Coach will provide referrals to: education and career programs such as: HSE, Vocational Training, Employment, and Internships. As well as: Substance Abuse Treatment, and Mental Health Services. Salary: $48,000 to $52,000 (base salary commensurate with education, training and related work experience) Responsibilities: • Conduct a comprehensive intake assessment for all new residents and review the agency’s policies and procedures with them. Develop treatment plan for all residents. Monitor clients’ emotional, social and physical well-being, as well as their progress • Maintain concise and accurate resident files in Salesforce database • Monitor residents’ medication • Provide residents with necessary referrals, including emergencies such as hospitalization, detox, etc. Inform residents about on- site, agency and community resources that are available to them. • Participate in case conferences, staff meetings and training sessions and represent the agency at assigned meetings and conferences. • Input data in Salesforce data base daily for all of the residents. • Attend professional development trainings, supervision meetings, case conferences, and staff meetings. • Provide monthly statistical reports on time and accurately. • Work cooperatively with staff at all levels to ensure quality service provision and the development and improvement of team performance. • Assist residents in the planning, prioritizing and long-term goals and short-term goals. • Maintain and oversee documentation of case files for compliance with regulatory agency. JOB DESCRIPTION: Direct Support Professionals work with individuals and/or small groups of Developmentally Disabled (DD) and/ or Intellectual Disabilities (DI) individuals. Provide instruction, supervision and direct assistance in facilitating services as outlined in individual’s Habilitative/ Staff Action Plan. Provide training and supervision in various activity of daily living, including: self-help, communications, interpersonal relations, leisure activities, etc. Transport program participants to and from their house and activities. Ensure that the vehicle is maintained clean and in proper working condition. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assist in the development of the individual’s Habilitative/ Staff Action Plan, Safeguards and in meeting that the objectives are in their plan of services. Assistance may be required in the following areas: cleaning, preparing snacks, social skills, budgeting, reading, math, computer skills, self-advocacy skills, volunteering, recreational and physical fitness activities. Staff may need to provide hands on demonstration, modeling, prompting, and instruction in the use of these (and other) skills. Completes all required Services billing documentation in an accurate and timely manner. Submit daily service notes accurately and contemporaneously. Report and document all incidents. Ensures that individuals’ goals are person centered and promote independence, inclusion and/or integration into the community. Transport program participants to and from their houses and activities. Ensure that everyone in the vehicle is wearing their seatbelt. Ensure that the vehicle is maintained clean and in proper working condition. Inspect the vehicle on a daily basis to ensure that safe to transport staff and program participants. Serve as a liaison between the parents and the program. Summary/objective: In Home Respite Direct Support Professional The Respite Support Worker works in the home to support people with moderate to very high support needs. The Support Worker provides direct care and support to people with disabilities in a variety of areas (ADL, meal preparation, light household cleaning, community inclusion, etc.). This may include persons of all ages with developmental disabilities who qualify for Services. Care taking may include providing meaningful interaction and activity for the client and providing protective supervision and/or respite for the family. Job duties/functions: 1. Undertaking health care procedures relating to the person’s personal hygiene, and wellbeing, such as toileting, showering and bathing, helping with dressing and grooming, manual handling, eating, nutrition and health. 2. Provide companionship and support during daily activities. 3. Perform various household tasks including cooking, cleaning, shopping and general services. 4. Encouraging and supporting consumer participation in social and community activities. 5. Maintaining consumer privacy and confidentiality at all times. 6. Maintaining a respectful and supportive working relationship with the client and family members. 7. Ensure the safety and rights of consumers is considered and maintained at all times 8. Program Development Services is seeking an Assistant Residential Manager reporting to the Residential Manager. The candidate will be assisting in supervising the day-to-day operations of the residence. Responsibilities include but are not limited to ensuring safety and sanitation in the residence and that adequate support services (vendors, suppliers) are maintained; providing direct services to the Individuals together with the direct care staff; supervising the DSPs, ensuring that training is provided to the facility staff. Requirements High School diploma/GED. Associate’s degree in Health or Human Services preferred. Minimum of two years of supervisory experience with the developmentally disabled population is preferred. Minimum two years related experience in a social service setting. Must be able to maintain strict confidentiality. Must be able to work a flexible schedule; evenings, weekends and overnights. Must have clean driving record and valid NYS drivers’ licenses.
