WE ARE HIRING RESIDENTIAL The Direct Support Professional (DSP) provides assistance to participants who need help with daily living skills, and other activities such as, activity development and training in social interaction. This position assists participants with bathing, feeding, toileting, laundry, and other self-care responsibilities. The DSP is responsible for safely accompanying participants to community resources, medical appointments, and other approved activities. The position is responsible for completing progress notes and making appropriate entries into the daily log book. FULL-TIME SHIFTS M-F 3PM-11PM, 4PM-12AM, 12AM-8AM, SAT AND SUN, 8AM-8PM, 8PM-8AM Medical Assistant/Direct Support Professional DRIVER LICENSE REQUIRED This position performs the duties listed under the DSP position in addition to assisting the participant with achieving and maintaining good health, provide support and help the participant achieve their goals in life, and live as independently as possible. They would also be responsible for scheduling and escorting the participants to medical appointments and acting as an advocate and liaison while on appointments. Administer medication, check MAR sheets and maintain participant medical records in compliance with OPWDD standards. Prepare case notes, read and make appropriate entries in facility logs daily. Review communication logs, Medication Administration Log and Medical Log at beginning of each shift. Participate as a member of the Interdisciplinary Team in the development, implementation and evaluation of the residents’ Plan of Care. Accompany participants to Cardio Club and participate in activities and carry out fire drills, evacuations and emergency procedures. **Part-Time Day Habilitation Driver/DSP** Qualifications •Must have at least a high school diploma or GED •Driver's License (Required) •Must be able to drive a 12-15 Passenger Van •High school or equivalent (Required) Driver for Day Habilitation/Respite Program: Monday-Friday 7-10am / 1:00pm-4:00pm / 4:00pm-6:00pm ● Transporting Developmentally Disabled individuals to and from their residences to the Day Habilitation program in Bronx,NY using an agency vehicle ● Arrive at destinations on schedule ● Ability to plan for routes: ○ Use navigation applications to determine the best route(s) ○ Be aware of traffic, weather delays ● Ability to develop a rapport with the individuals of various backgrounds ● Must abide by OPWDD rules and regulations ● Ensure the agency vehicle is always in proper working order: ○ Fueled and ready to use ○ Report any vehicle maintenance needed to supervisor ○ Vehicle interior is kept clean ● Maintaining mileage record and Pick-up/Drop-Off logs Required Education and experience: ● Clean and valid New York State Driver’s License ● Excellent verbal and written skills ● Interact with individuals professionally and respectfully ● Ability to pass a background check ● Commercial Drivers License a plus ● Experience providing direct care of services to adults with developmental disabilities ● CPR & First Aid trained a plus as well

Summary/objective: In Home Respite Direct Support Professional The Respite Support Worker works in the home to support people with moderate to very high support needs. The Support Worker provides direct care and support to people with disabilities in a variety of areas (ADL, meal preparation, light household cleaning, community inclusion, etc.). This may include persons of all ages with developmental disabilities who qualify for Services. Care taking may include providing meaningful interaction and activity for the client and providing protective supervision and/or respite for the family. Job duties/functions: 1. Undertaking health care procedures relating to the person’s personal hygiene, and wellbeing, such as toileting, showering and bathing, helping with dressing and grooming, manual handling, eating, nutrition and health. 2. Provide companionship and support during daily activities. 3. Perform various household tasks including cooking, cleaning, shopping and general services. 4. Encouraging and supporting consumer participation in social and community activities. 5. Maintaining consumer privacy and confidentiality at all times. 6. Maintaining a respectful and supportive working relationship with the client and family members. 7. Ensure the safety and rights of consumers is considered and maintained at all times 8. Write accurate progress notes in the consumer’s file following each occasion of support. 9. Maintain a record of hours worked on the time sheet provided. 10. Adhere to the identified safe systems of work and report any accidents, incidents or hazards promptly to your supervisor. 11. Participate in required training as organized by Sinergia. Program Development Services is seeking an Assistant Residential Manager reporting to the Residential Manager. The candidate will be assisting in supervising the day-to-day operations of the residence. Responsibilities include but are not limited to ensuring safety and sanitation in the residence and that adequate support services (vendors, suppliers) are maintained; providing direct services to the Individuals together with the direct care staff; supervising the DSPs, ensuring that training is provided to the facility staff. Requirements High School diploma/GED. Associate’s degree in Health or Human Services preferred. Minimum of two years of supervisory experience with the developmentally disabled population is preferred. Minimum two years related experience in a social service setting. Must be able to maintain strict confidentiality. Must be able to work a flexible schedule; evenings, weekends and overnights. Must have clean driving record and valid NYS drivers’ licenses.
