Our Prevention program delivers a range of clinical, case management, and supportive services to families who are child-welfare involved and caring for children diagnosed with chronic or complex medical conditions. The goal is for families to remain intact while ensuring caregivers receive the appropriate support to meet their children's physical, developmental, and mental health needs. The ideal candidate will be strength-based, trauma-informed, culturally responsive and committed to family preservation and resilience. This is a city-wide home and community-based program, and the candidate must have the ability to work independently and flex their hours on weekdays to meet the needs of youth and caregivers. Our hybrid model supports work-from-home when staff are not in the field. Additional compensation for bilingual skills and licenses (LMSW, LMHC, etc.), • Location: 825 7th Avenue, New York, NY, 10019, • Department/Program: Special Medical Prevention Services Description of Responsibilities: • Social Worker/Mental Health Counselor provides casework and clinical services to youth and caregivers utilizing individual and family modalities. Coordinates services for a caseload of 8-9 families as recommended by NAC's multidisciplinary treatment team., • Conducts home and community-based visits., • Adheres to agency and ACS regulatory requirements related to casework and collateral contacts, Family Team Conferencing, Workforce Institute training, and timely completion of Family Assessment and Service Plans (FASPS) and progress notes in CONNECTIONS., • Social Worker/Mental Health Counselor makes adequate use of individual, group and peer supervision, clinical consultation, mental health in-services, and social justice discussion forums. Candidates receive credit toward NYS LCSW and LMHC licensing requirements. Free CEUs are available to licensed social workers (LMSW/LCSW) along with licensing prep courses for MSWs and MHCs who have completed probation., • Social Worker/Mental Health Counselor collaborates with psychiatrists, psychologists, pediatricians, nurses, case associates, and resource and education specialists in a dynamic, strength-based interdisciplinary team model. About the Role The Unit Counselor supports residents by providing care, support, socialization, crisis intervention, and facility monitoring across shelter programs. Primary Responsibilities • Participate in or conduct orientation for new residents., • Communicate program processes and activities to residents., • Provide social support and motivational interviewing interventions., • Provide transportation for residents to appointments as needed., • Assist with meal-related activities including warming, serving, and cleanup., • Document neighborhood activities and resident interactions., • Participate in crisis intervention as needed., • Attend management, supervision, and required training meetings., • Report serious incidents and complete required incident documentation., • Conduct hourly rounds and ensure resident wellbeing., • Arrive to work on time and as scheduled., • Perform other job-related duties as assigned.
