RESPONSIBILITIES The Social Worker will manage clinical cases for caseload of residents receiving HRA HASA Supportive Housing Services in addition to seniors, low income working or disabled residents identified has having significant mental health and/or substance abuse issues. Social Worker will assist clients in maintaining medical, emotional and housing stability, utilizing counseling interventions and appropriate referrals and maintain a consistent schedule of face to face client contacts. This includes orientation, interventions & counseling to address conflict resolutions when roommate conflicts arise.
