Job Description Overview Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Responsibilities The Pharmacy Technician performs all technical tasks in the unit dose, carry and out-patient drug distribution systems as well as other related duties of a satellite pharmacy. The Registered Pharmacist approves all work performed by the Pharmacy Technician. • Unit Dose and Carry Drug Distribution Systems, • Assists in maintaining accurate patient profiles by informing the pharmacist of changes in patient names, I.D. number or housing so modifications can be made in the pharmacy database., • Fills and labels patient cassettes with non-carry and carry system medication bags from computer generated fill lists, and/or original drug orders., • Obtains court medication list from General Office. Reviews distribution list to see if patient will be getting medication in morning. Reviews what is in Pyxis. If medication is not in Pyxis, provides dose for morning court medications. If medication is not in Pyxis, technician can have it added to Pyxis., • When outside methadone clinics call to verify last dose and date of methadone, technician will review Accumed methadone administration records to ascertain the patient’s last dose and date of methadone administration., • If there are requests for a patient profile, the individual should be instructed to get the profile themselves from report manager. Requests from other agencies should be referred to the pharmacist on duty., • Prints and reviews all transfer reports, checks if the patients is on medication, prints patient medication profile and fills needed doses of medication for subsequent checking by pharmacist. If patient is on methadone or DOT medications, tells pharmacist of such., • Transports patient cassettes to medication clinic areas and exchanges same when necessary., • Files completed fill lists, updates lists, distribution lists, methadone sheets and other materials according to procedure., • Checks, rotates and restocks all drug stock areas. Fills Pyxis machines as required. Files Pyxis reports with non pyxis stock records and retains them for 1 week. On a monthly basis checks for outdated medications in Pyxis, clinic floor stock, pharmacy stock and other areas where pharmacy stores medications. For non pyxis floor stock, maintains established par levels based on individual clinic or infirmary need. Records issues of stock on the non pyxis Floor Stock Inventory and Issue Record., • Delivers emergency boxes and other requested items to clinic or infirmary areas as needed., • Prepares weekly pharmacy stock order by utilizing par levels and building usage patterns for review by the pharmacist. Enters order into IDWhere and transmits on both a daily and weekly basis to packaging. Reconciles packing slip or invoice against items received. Notifies building pharmacist and packaging manager of any discrepancies. Puts away orders upon receipt, appropriately rotating stocks as required., • Inspects all drug storage areas (clinic, mini-clinic, urgi-center, infirmary, pharmacy, par level rooms, dental, X-Ray, etc.) for outdated medication, IV solutions and related supplies on a monthly basis., • Sorts, counts, prepares outdated drug manifest and packs up outdated medications for return to Central Medical Supply for submission to a reverse distributor., • Prepares and records the prepackaging of extemporaneous unit dose medication., • Records daily statistics on the RX Daily Stats Activity Report form in the computer. Gathers and records other statistical and survey information as required. Emails to appropriate party as requested., • Has a working knowledge of the pharmacy and medical computer systems. Keys data into the pharmacy system (compliance and OTC requests) and retrieves data from the computer as directed by the pharmacist and permitted by law. Prints or reprints all reports/labels as required., • Maintains all pharmacy areas, including “bubbles,” in a clean, orderly and well stocked fashion for all shifts. Helps arrange for sanitation pick-up and cleaning of these areas with the Department of Correction., • Allows new personnel to shadow them., • Monitors the quality of patient care through quality assurance/improvement mechanisms. Can remind patients that they are on medications and how to pick up the medications. Can refer patients to pharmacist or sick call for additional information. Can compare daily output of medical computer and pharmacy computer and notify users of missing data., • Distributes medication that has been checked and approved by the pharmacist to the correctly identified patient for self-administration in the proper way and at the proper time. Records receipt of the medication by the patient by clearly initialing and recording the number of days’ supply given on the MDR and by computer entry. (As described in the Medication Distribution Policy.), • Refers to the pharmacist or clinic any patient who requires further information concerning their medication, is non-compliant, or is having difficulty tolerating their medication., • Contacts the clinic captain, control room or tour commander in the event of DOC related delays in distributing medications. Contacts on-island administrator in the event of a delay or any other occurrences or problems which adversely affects the above procedures. Once a problem has been reported, keeps the on-island administrator informed about either the progress or lack thereof in distributing medication. (As described in the Disruption of Services Policy.), • Sets up and operates liquid and solid unit dose repackaging machines. Responsible for the proper maintenance and cleanliness of the machine at the end of each run., • Packages oral solids into plastic zip-lock bags for the carry system., • Maintains proper record keeping and control procedures as outlined in the Packaging policy. Job Description Position Summary The Work Order Coordinator will play a critical role in ensuring efficient maintenance and repair operations for our managed properties. This program is part of the Atlas initiative and uses the evidence-based Functional Family Therapy (FFT) model to engage young people and their families in holistic anti-violence services. Reporting to the Justice Center’s FFT Coordinator, the Community Engagement Specialist will be primarily responsible for conducting initial outreach in the community in multiple ways, including in-person. The Community Engagement Specialist is the first and primary point of contact for potential referral partners. The Community Engagement Specialist will also assist with connecting potential clients with voluntary therapeutic programming and resources. Participants are either mandated or encouraged to voluntarily attend this class as part of their supervised release program. The class offers participants an overview of basic principles of power and control dynamics and uses an interactive format to introduce strategies for making healthy choices in intimate relationships. The Bilingual Facilitator will provide this class as part of a larger IPV supervised release track, with the goal of increasing compliance with court mandates and return to court. The Bilingual Facilitator will also train in other IPV and domestic violence groups and classes as the team seeks to expand programming. The Bilingual Facilitator will primarily teach online but will is also required to teach classes in person throughout New York City's five boroughs as the program expands. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. THE OPPORTUNITY Brooklyn Justice Initiatives (BJI), the largest operating project of the Center, seeks to re-engineer the experience of criminal court in Brooklyn, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Kings County Criminal Court and community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. BJI seeks Case Managers for the borough’s Supervised Release Program (SRP). Reporting to Supervising Case Managers, Case Managers are the front line of service delivery and key to ensuring that the program meets its mandate. Specifically, Case Managers will supervise SRP participants according to their assigned tier and level; work with participants to identify and address barriers to compliance and court attendance; conduct outreach to participants, court stakeholders, community-based service providers, and collateral contacts; connect participants to appropriate services in the community to support their stabilization, and ensure accurate and timely court reporting on participants’ compliance with program requirements. Accurate and timely data entry is a core responsibility for this position. Additionally, Case Managers will co-facilitate pro-social and psycho-educational groups. THE OPPORTUNITY Brooklyn Justice Initiatives (BJI), the largest operating project of the Center, seeks to re-engineer the experience of criminal court in Brooklyn, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Kings County Criminal Court and community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. BJI seeks Case Managers for the borough’s Supervised Release Program (SRP) Brownsville/East New York office. Reporting to Supervising Case Managers, Case Managers are the front line of service delivery and key to ensuring that the program meets its mandate. Participants are either mandated or encouraged to voluntarily attend this class as part of their supervised release program. The class offers participants an overview of basic principles of power and control dynamics and uses an interactive format to introduce strategies for making healthy choices in intimate relationships. The Facilitator will provide this class as part of a larger IPV supervised release track, with the goal of increasing compliance with court mandates and return to court. The Facilitator will also train in other IPV and domestic violence groups and classes as the team seeks to expand programming. The facilitator will primarily teach online and but will also teach classes in person throughout New York City's five boroughs as the program expands. The class will be offered in several separate groups: Spanish-speaking, male, female, and LGBTQ+ defendants utilizing different curricula. Classes are offered in other languages with an interpreter on an as-needed basis. Save Our Streets South Bronx is a replication of the Chicago Project for Violence Prevention’s evidence-based Cure Violence program, a cutting-edge gun violence prevention model that aims to reduce and prevent shootings through the use of public health strategies. The Bronx Community Justice Center is seeking an Outreach Worker for the Save Our Streets South Bronx program. Reporting to the Outreach Worker Supervisor, the Outreach Worker will serve as a mentor and peer counselor to a caseload of 10-15 individuals most impacted by community violence, redirecting them toward positive attitude, behavior, and lifestyle changes. THE OPPORTUNITY Brooklyn Justice Initiatives (BJI), the largest operating project of the Center, seeks to re-engineer the experience of criminal court in Brooklyn, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Kings County Criminal Court along with community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming and receive referrals to services such as job training, drug treatment, and mental health counseling. BJI is seeking a Supervising Case Manager for the borough’s Supervised Release Program (SRP) to supervise our case management and field engagement staff who are based in the community. Reporting to the Case Management Coordinator, the Supervising Case Manager will be responsible for providing substantive and consistent trauma-informed supervision to Case Managers, Senior Case Managers, and Community Navigators to ensure fidelity to the program model and quality service to participants in the community. Supervising Case Managers ensure that staff complete program-specific training, consistently apply policy, protocol, and best practices in alignment with that training, maintain routine and accurate data entry and court reporting, and collaborate effectively across SRP teams to build on successes and proactively address challenges. This position is also responsible for completing comprehensive and routine case audits in partnership with the compliance team to ensure consistent application of policies, protocols, and best practices. The Behavioral Health Center offers a comprehensive suite of services for mental health including First Episode Psychosis program, medical services, opioid treatment, substance use disorders services, a specialized unit for people with Intellectually Disability, an inpatient Extended care unit focusing on the needs of our homeless patients and prevent recidivism, ketamine assisted therapy, and inpatient and outpatient services for adults and children. Our skilled team—including adult and child and adolescent psychiatrists, psychologists, internists, nurses, social workers, creative arts therapists, peer counselors and addiction counselors delivers compassionate care in a secure environment, respecting each patient's dignity and partnering with them on their recovery journey. Our services range from psychopharmacology and various forms of psychotherapy (individual, group, and family) to specialized programs like outpatient withdrawal management, medication maintenance (including Methadone and Buprenorphine), overdose prevention, and support for personal development goals such as employment and education. Each patient receives a personalized assessment and treatment plan tailored to their specific needs, ensuring a holistic approach to recovery and health management. Responsibilities The Unit Chief is the administrative and clinical lead of the assigned inpatient unit and is responsible for all aspects of patient care on the unit. They work with the inter-disciplinary teams to ensure safety and quality on the inpatient unit. Oversee inpatient structure • Establish and oversee inter-disciplinary teams and team meetings, • Ensure that community meetings and other therapeutic groups take place as scheduled, • Participate in Behavioral Codes on the unit, providing leadership to the staff and patient community, • Work with all teams to monitor length of stay, particularly ensuring that, • Patients who are refusing medications are taken to court promptly, and that, • Patients who have reached their 18th day are referred for state hospitalization for continuing treatment, • Identify patients who would benefit from AOT and work with their team to ensure that the application is completed., • Work with medical-consultation team ensure that medical problems are properly managed, • Ensure that legal statuses are up to date and accurate, • Provide clinical consultation to other team (when present) on your unit, • Provide assistance to other psychiatrist (when present), as needed, • Monitor the need for psychiatrist assessments and other components of applications of aftercare, in such applications for appropriate housing, residential placements and state referrals, • Monitor chart for completion and clinical accuracy, • Participate in Performance, Quality Assurance and Clinical pertinence activities, including chart review, and educate inpatient staff members on Performance Improvement, Quality Assurance activities, indicators, etc. and be able to help the unit improve if their performance is below the mean for the service, • Restraints, • Manual holds, • Assist in the preparation for surveys, • Work with the UM department to conduct MD to MD reviews and participate in any appeals to ensure authorization of inpatient stay., • Maintains focus on key strategic priorities including patient experience, access to care, increased market share, and improving financial stability Qualifications • Must be board certified in Child Psychiatry, • M.D. or D.O. from an accredited school of medicine with License, • Completed residency training in an accredited program within Psychiatry Annual Base Salary: $267,000 based on 40-hour work week.
