Job Description Supervisor of Client Services (Case Manager) Summary: Responsible for collaborating with a variety of agencies and service providers in the community on both a professional and paraprofessional level to provide housing and supportive services for program participants. Responsible for providing recovery based, client centered support services using the most effective available resources in meeting the identified service plan goals for each client. Position Type: Full Time, Non-Exempt Essential Functions: • Provide supervision for program participants including coordinating and monitoring the activities of all other service providers for each client. Responsible for assessment of client’s strengths and needs. Develops a service plan within no more than 5 days of admission to the program in conjunction with the client which will be updated at least every 6 months or as changes occur. Educate and support client family and advocate for client rights and preferences., • Maintain current, accurate, and thorough documentation of at least two face to face contacts in client charts per week., • Develop relationships with representatives in other agencies to support individuals in attaining services such as housing, additional mental health care, psychiatric medication, medical resources, financial assistance, legal advocacy, etc., • Meet with each client twice per week in their home or community setting (call must be made to if the person is offsite)., • Participate in staff meetings., • Monitor for medication adherence and educate client on medication side effects, dosing, and provides oversight for organizing pill boxes., • Responsible for obtaining required hours of training per fiscal year., • Adhere to all client confidentiality requirements and standards., • Maintain and submit proof of active auto liability insurance for privately-owned vehicles used to conduct agency business and/or transport agency clients. Employees must submit proof of insurance prior to employment and each time policy is renewed thereafter. Failure to maintain active coverage and submit proof may result in disciplinary action., • Transport clients as directed and follow agency standards, rules, regulations, and all laws for safe transport., • Direct all questions to Housing Supervisor if unsure of how to proceed with a client or how to document activities., • Meet regularly with Housing Supervisor for case review and update., • Conduct pre-entry and eligibility screenings for all referral for disability and homeless criteria., • Monitor and verify participant income and calculate rental payments on at least annual bases., • Coordinate the client admission process and treatment, rehabilitation, and support services of the program in coordination with the Housing Supervisor and Mental Health Provider. Schedule the admission interview, develop, and coordinate the initial assessment in coordination with the Housing Supervisor., • Monitor completion and or complete the intake documentation within no more than two days of client admission into the program. If intake is not completed within less than two days, written justification needs to be provided in the chart and to Housing Supervisor., • Communication Proficiency, • Accountability for Others, • Balanced Decision Making, • Leading Others, • Flexibility, • Initiative, • Analysis of Data, • Problem Management Supervisory Responsibility: No Required Education and Experience: Bachelor’s Degree in a human service related field preferred and one to three years of experience in direct services with individuals with disabilities. The individual must possess a working knowledge of community resources and excellent communication and interpersonal skills. Computer skills and knowledge of Microsoft desirable. Physical Demands: While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
