Weare currently seeking a dedicated and compassionate Primary Care Nurse Practitioner (NP) to join our team and work closely under the supervision of our Family Medicine Physician. Job Summary: We are seeking a compassionate and skilled Primary Care Nurse Practitioner (NP) to join Hamilton Medical Group under the supervision of Dr. Ashley West, a board-certified Family Medicine Physician. The NP will provide high-quality, patient-centered primary care services to adults, collaborating closely with a collaborating physician to ensure comprehensive and coordinated care. This role involves diagnosing and treating a wide range of acute and chronic conditions, promoting preventive care, and fostering strong patient relationships in a supportive, team-oriented environment. Key Responsibilities: • Conduct comprehensive patient assessments, including medical history, physical exams, and diagnostic evaluations., • Diagnose and manage acute and chronic illnesses, such as infections, diabetes, hypertension, and respiratory conditions., • Order, interpret, and follow up on diagnostic tests (e.g., lab work, imaging)., • Prescribe medications and therapies in accordance with state regulations and practice protocols., • Provide preventive care, including health screenings, immunizations, and patient education on wellness and disease prevention., • Collaborate with Dr. Ashley West to develop and implement individualized treatment plans., • Document patient encounters accurately and timely in the electronic health record (EHR) system., • Coordinate referrals to specialists and other healthcare providers as needed., • Participate in patient follow-ups and care coordination to ensure continuity of care., • Educate patients and families on health conditions, treatment options, and lifestyle modifications., • Adhere to evidence-based guidelines, clinic policies, and state regulations governing nurse practitioner practice., • Contribute to quality improvement initiatives and team meetings to enhance patient care delivery. Qualifications: • Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program., • Current, unrestricted Nurse Practitioner license in Louisiana., • National certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)., • Minimum of 1-3 years experience in primary care or family medicine preferred; new graduates with strong clinical training may be considered., • Current BLS certification, • Proficiency in EHR systems and basic computer skills., • Strong clinical assessment and diagnostic skills., • Excellent communication, interpersonal, and teamwork skills., • Commitment to patient-centered care and cultural sensitivity.
