Job Description The DSP (CFASS, Respite Or Prevocational) service provider provides community-based services that focus on short-term assistance provided to children regardless of disability, because of the absence of or need for relief of the child, or the child’s family caregiver. The provider will supervise the child/youth and engage the child/youth in activities. Providers will offer services with a level of expertise in understanding and implementing behavioral/developmental interventions required to support optimal functioning for children/youth during planned care. Essential Duties and Responsibilities Training for the child/youth and/or the family/caregiver regarding methods and behaviors to enable success in the community. Direct self-advocacy training in the community with collateral contacts regarding the child/youth’s disability(ies) and needs related to his or her health care issues. Self-advocacy training for the child/youth and/or family/caregiver, including during community transitions. Improve the child/youth’s ability to gain from community experience. Updating of Person-Centered service plans (every 6 months) including obtaining medical consenter signatures at the initial session following the approval of such documentation. Attend monthly waiver service provider meetings and individual’s supervision to stay current on practices and protocols. Submit progress notes, service plans, and service provider logs within set forth-required timeframes. It is the responsibility of the provider to maintain necessary technology (phone, internet, timely computer access, and video camera) to perform job requirement functions of documentation, coordinating, service planning and billing. Report incident(s) to supervisor upon discovery. Maintain open communication with other service providers (e.g., Care Manager, other service providers and Supervisory Team) to report on child/youth’s progress, identify any potential barriers and referrals, and to support the overall health and wellbeing of the child and family. Check emails daily and respond to emails from Care Managers, Supervisors, and other team members within 24-48 hour. Provide short-term relief for the child or family/primary caregivers that are needed to enhance the family/primary caregiver’s ability to support the child/youth’s functional, developmental, behavioral health, and/or health care needs. Planned Respite activities support the plan of care goals and include providing supervision and activities that match the child/youth’s developmental stage and continue to maintain the child/youth’s health and safety. Setting of Service Provision: Services can be provided in a variety of settings, including community locations or most frequently in the family or caregiver’s home. Essential Skills, Experience, Qualifications: Respite providers are minimally required to have a high school diploma or equivalent and with appropriate skills and training. Experience working with children/youth. RESPONSIBILITIES • Primary function is to provide community outreach, engagement, awareness, education, and resources, while bridging the gap between the Jobs Plus program and the communities we serve., • Act as an intermediary to facilitate communication between BronxWorks and targeted NYCHA residents and members to raise public awareness and promotion of the Jobs Plus program employment related services, activities, resources, income supports, support services (i.e. legal assistance, mental health counseling, emergency food) ,benefits and incentives (i.e. Earned Income Disallowance)., • Promote and distribute program marketing materials (i.e. flyers) within and around the developments through a combination of knocking on doors, talking to residents in their homes or lobbies, and attending tenant association meetings, community meetings or events., • Help to advertise upcoming events and program offerings on social media, • Ensure that messages are consistently delivered, allowing for an open dialogue between BronxWorks and the community., • Develop a better understanding of what are the target audience needs and wants to enable the program to provide more relevant services and solutions that meet their audience’s needs., • Responsible for developing and maintaining professional relationships between BronxWorks, partner organizations, and its various stakeholders (i.e. NYCHA REES, property management, etc.), • Collaborate to organize outreach meetings and outreach activities/events, • Attend community events, such as Tenant’s association meetings, family days, and recreational events. This may also include community board meetings, and NYPD Public Service (PSA) meetings, national night out, etc., • Other duties as assigned. QUALIFICATIONS • High school diploma/GED and at least 1 year experience working in social services. In lieu of a degree, 2 years relevant experience preferably in social services., • To be eligible for this position, you must be a resident of New York City Housing Authority (NYCHA) of the following 8 developments: Melrose, Betances, Courtlandt, Moore, Sack Wern, Soundview, Clason Point Gardens and Monroe., • Exceptional communication skills, strong organization and interpersonal skills and qualities., • Knowledge and proficiency of Microsoft Office suite and other standard business technology is required., • Data management and tracking is required. POSITION CRITERIA: • Leadership skills and self-starter, • Previous outreach or community engagement experience as a community liaison or organizer is a plus, • Must be comfortable working in intercultural settings., • Have a wealth of knowledge to strategize engagement opportunities with the targeted community areas of Soundview and South Bronx., • Strong qualities that will enable to effectively engage with, support community outreach, and build positive relationships within their community., • Have adaptability and problem-solving skills to be productive in order to navigate unexpected situations, handle conflicts, and find creative solutions to challenges that arise within the community., • Must have empathy and understanding when communicating with community partners and leaders., • Ability to relate and connect with community members by maintaining professional relationships and demonstrating empathy for their concerns and challenges; while building trust and establish constructive relationships within the community., • Strong use of the English language in verbal and written communication. Proficiency in Spanish preferred PHYSICAL REQUIREMENTS • Ability to use a computer for prolonged periods, • Ability to occasionally lift and/or move up to 10 pounds., • Ability to stand, walk, or sit for long periods of time., • Ability to bend and retrieve objects and/or documents.
