Job Description Summary This position is responsible for overseeing the management and safeguarding of medications within a residential treatment program. This position provides support and oversight for resident’s self-administration of medication while in the residential treatment facility. Essential Duties and Responsibilities 1. Conduct admission assessments of residents: health history, vital signs, signs/symptoms of withdrawal from substances, signs/symptoms of mental health disorders, risk assessment, and need for urgent or emergent care., 2. Responsible for receipt and inventory of medication, creating and updating documentation necessary for the storage of an oversight in utilization of medications at the program., 3. Complete consent forms and referrals as necessary to meet the physical health, mental health, and substance abuse treatment needs of residents., 4. Connect residents with health care provider(s) for completion of admission required physical examination and TB clearance, as well as ongoing care as needed to meet the individual needs of residents., 5. Verify resident’s medical insurance information (Medi-Cal and other third-party payors) and coordination efforts with pharmacy., 6. Maintain a clean, orderly, medication room with properly functioning doors, cabinets, and appropriate room and refrigerator temperature, making timely reports to management to resolve any issues., 7. Ensures that a medication inventory is conducted weekly and medication(s) with a ten-day supply or less is/are ordered for refill, or resident is scheduled for follow-up appointment with prescribing provider., 8. Manage the timely ordering/receipt of medications, proper storage, proper destruction of medications, and the documentation required for these activities., 9. Track, document, implement, and communicate medication changes and medication related information to treatment team and supervisor., 10. Review for accuracy of medication prescription orders prior to publishing monthly medication logs for resident’s self-administration of medication., 12. Coordinate with prescribing practitioners, pharmacies, and treatment team in the provision of mental health services, physical health services, as well as medication and pharmacy related services., 14. Collect and manage data on MAT services, IMS utilization, overdose prevention/reversals and harm reduction efforts., 17. Maintain all medication and MAT training records of staff., 18.
