Job Description Role and Responsibilities The Therapy Field Staff Supervisor (TFSS) is an integral part of the patient care team and continually monitors therapy services provided. This role has shared responsibility for the overall provision and coordination of home health services provided to the HCR patients, according to acceptable standards of practice. The TFSS is a resource person with extensive knowledge of the regulatory guidelines governing home health agencies and provides daily support to field clinicians enabling them to provide quality care in the home to HCR patients. The TFSS is a liaison between referral resources, patients, caregivers, and employees. Essential Functions • Act as liaison to assist in appropriate ongoing management of care; ensure appropriate plans and interventions are identified, act as resource for problem solving or advising on patient care questions or concern., • Complete review of Physician ordered Plan of Care (POC)/485 and OASIS Assessment to ensure completeness and that they accurately reflect patients’ condition and needs., • Complete review of Resumption of Care (ROC) and discharge assessment data and approved orders, as needed., • Review and complete all assigned HCHB-related tasks and coordination note workflow on a daily basis, based on set timeframes. Communicate with supervisor when unable to complete timely., • Enter new patient orders as appropriate and update the plan of care and patient calendar as appropriate., • Review and approve all new interim orders before they are sent to physician. Our focus is to develop a supportive schedule that meets both the needs of our patients and the providers' preferences. Your schedule will be determined with you before starting the role. Our operating hours are M-F: 8 am to 8 pm and Saturday: 9 am to 5 pm, but we do allow a schedule to have a start as early as 7 am, and end as late as 9 pm M-F.In-office day is based on availability if hybrid work is preferredInclude at least four(4)+ hours within your schedule, which should be after 5 pm on a weekday or have hours on the weekend. On-call is not required, but checking and responding to emails on unscheduled days is required unless on PTO.Having a consistent weekly schedule is necessary, but start and end times per day can be different each day of the week, as a set recurring schedule. We appreciate the dedication and care our providers bring to their work and the patients they serve. Job Description Job Summary Under the direction of the Director of Menorah Park Group Residences, the Residence Counselor is responsible for working with Consumers to foster a nurturing, positive and supportive living environment while following Consumer Service Plans to attain/maintain the Consumers highest level of independence and meeting Service Plan goals. Key Job Responsibilities Residence Counselor Consumer support: Supports Consumers and provides direct oversight with completing personal hygiene, including bathing, grooming, oral care, toileting, dressing and undressing, along with communication, interpersonal and leisure actives. Assists and/or monitors therapeutic activities such as physical exercises, occupation arts and crafts, and recreational games. Follows the consumer service plan goals to attain the highest level of independence possible. Accompanies and transports individuals on shopping and recreation trips and provides instruction and assistance as needed. May be required to provide transportation in personal vehicle on occasion. • Feeding Assistance: Help consumers maintain nutritional needs, including meal prep, set up, clean up, feeding assistance as needed, and monitoring and documenting dietary intake., • Medication Administration: Administers, supervises and/or monitors administration of medications as prescribed by physician., • Observation and Reporting: Observe residents for any changes in condition and promptly report these to the Director.
