Job Description ob post summary Date posted: December 4, 2025 Pay: $60.00 - $75 per visit Job description: Job Overview We are seeking a passionate and dedicated Occupational Therapist to join our home health team, providing exceptional patient-centered care in patients’ homes. In this vital role, you will assess, plan, and implement therapeutic interventions to help individuals regain independence and improve their quality of life. Your expertise will support diverse populations, including pediatrics, geriatrics, individuals with disabilities, and those recovering from acute or post-acute conditions. This position offers an exciting opportunity to make a meaningful difference in patients’ lives while working in a flexible, community-based setting. Duties • Conduct in-home OT evaluations to assess ADL performance, cognition, fine motor skills, coordination, and strength., • Develop individualized treatment plans tailored to patient goals, medical conditions, home environment, and functional ability., • Implement evidence-based interventions including ADL retraining, energy conservation, neuro-motor rehab, balance training, visual-perceptual tasks, adaptive equipment use, and home safety modifications., • Train patients and caregivers in transfers, fall-prevention, compensatory strategies, and equipment use such as grab bars, shower chairs, and assistive devices., • Provide home safety evaluations and recommend modifications to support aging-in-place., • Document all visits, progress notes, goals, and reassessments in Winnolas EMR., • Collaborate with Skilled Nursing, PT, Speech Therapy, Social Work, and Aides to coordinate a unified plan of care., • Strong knowledge of physiology, anatomy, and medical terminology relevant to occupational therapy practice, • Experience working across various settings such as inpatient units (including ICU), outpatient clinics, skilled nursing facilities, and home health environments, • Proficiency in discharge planning and case management to facilitate optimal patient outcomes, • Ability to assist with ADLs and functional activities for patients of all ages—pediatrics through geriatrics, • Familiarity with early intervention programs and school-based occupational therapy services, • Experience working with people with developmental disabilities, • Skilled in developing tailored care plans that address individual needs in post-acute care settings, • Ability to work effectively within multidisciplinary teams including speech therapists, physical therapists, nurses, and physicians Job Types: Part-time, PRN, Contract Work Location: In person.
