Job Summary This position is responsible for providing case management and referral services that assist residents in moving in a planned manner towards achieving personal independence. Job Duties/Responsibilities To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below. Reasonable accommodation may be provided to enable individuals with disabilities to perform the essential duties/responsibilities of this job. • Conduct Intakes with all referred families to determine their qualification for the SMS Shelter Programs., • Once a client is accepted into an SMS housing program, work with the family to assess barriers to obtaining housing., • Assist clients in developing an individualizedclient-driven Housing Stability Plan (Case Plan) that will enable them to obtain and retain stable permanent housing., • Create a case management file for each family, documenting case management services received and progress toward goals., • Ensure project outcome requirements are met for individuals and the program as a whole. This will vary dependent on project guidelines., • Meet regularly with clients to review progress toward goals and identify current action steps needed to achieve goals., • Act as an advocate for clients with other community agencies., • Provide referrals and information related to problem-solving, conflict resolution, parenting and interpersonal concern to assist clients in achieving personal goals., • Assist residents as needed with urgent care issues., • Assist in maintaining the cleanliness of the facility as needed., • Ensure that house rules are followed., • Ensure that donations are sorted and distributed according to Pearl guidelines., • Enter data into HMIS database and Salesforce at enrollment, exit and follow up, as required; provide monthly reports documenting activities for funding sources., • Carry out shift responsibilities to ensure the ongoing safety, cleanliness and smooth operation of St. Margarets facility. This includes flexibility in schedule with a 24/7 facility., • Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA)., • Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor., • As a mandated reporter, follow all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect., • Adheres to the tenets of Catholic Social Teaching and Catholic Doctrine., • Perform as a team member to ensure that productivity outcome measures are achieved., • Perform related functions necessary to support the mission and core values of Catholic Charities. Job Qualifications To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job. Education/Experience: to perform this job successfully, an individual must have: BA in the field of social services or equivalent experience. Knowledge and/or experience in providing case management services; experience in direct client services. Experience working with issues of homelessness, poverty and violence against women and children preferred. Certificates/Licenses: To perform this job successfully, an individual must have a valid Drivers License and the ability to drive for work use. Successfully pass background check applicable to position. Workplace Environmental Conditions While performing the essential duties/responsibilities of this job, the employee will be: • Noise Conditions: exposed during a shift to constant or intermittent sounds at moderate to loud levels of sound dependent upon the activities of the Shelter., • Exposure to Trauma: Employees of Catholic Charities Eastern Washington provide direct services to vulnerable community members including those who have behavioral health challenges and those who have been exposed to trauma.
