Uses cognitive-behavioral techniques (e.g. role-plays, skills training, plans of action) that assist the consumer in meeting service plan goals Examples of skills training include self-advocacy training, ADL (Activities of Daily Living) skill building, medication management training, socialization skill enhancement, and conflict resolution training. Functions as criminal justice liaison for consumer., • Plans, escorts, and/or arrange social/recreational, vocational, medical and other activities for and with consumers. Obtains and develops resource information for consumers in all aspects of their care (clinical services, recreational activities, vocational programs both within and outside of ICL and disseminates this information to consumers in a manner that is clear and understandable. Accounts for consumers and files missing person reports on consumers not accounted for in accordance with ICL policy and procedure., • Performs room checks and regular inspections of the consumer's rooms, using checklists or other aids, to ensure the safety of the consumers, accountability of ICL property, and cleanliness of consumer's residence. Engages consumer in learning how to maintain a safe, clean, and healthy personal living environment. Job Description New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency in Midtown Manhattan, dedicated to serving children and families with medical complexities, chronic illnesses, and disabilities throughout the five boroughs. To assist in achieving this mission, NAC is currently seeking a Social Worker/Mental Health Counselor to join its Special Medical Prevention Program. Our Prevention program delivers a range of clinical, case management, and supportive services to families who are child-welfare involved and caring for children diagnosed with chronic or complex medical conditions. The goal is for families to remain intact while ensuring caregivers receive the appropriate support to meet their children's physical, developmental, and mental health needs. The ideal candidate will be strength-based, trauma-informed, culturally responsive and committed to family preservation and resilience. This is a city-wide home and community-based program, and the candidate must have the ability to work independently and flex their hours on weekdays to meet the needs of youth and caregivers.
