The Clinical Care Manager (CCM) is responsible for delivering integrated, whole person care management to individuals with complex behavioral needs. The CCM facilitates care coordination across multiple systems, ensures timely utilization management (UM) activities, and promotes collaboration between providers, members, and support systems to improve health outcomes. Clinical Care Managers are assigned to specific teams based on population needs and service settings, with some responsibilities tailored to those contexts. Job Description The IBHS Clinical Care Manager – Neurodevelopmental Services (CCM-ND) is responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, provider management and treatment quality monitoring. The Clinical Care Manager must utilize clinical knowledge of best practices and evidence-based treatments, including Applied Behavior Analysis, for individuals with Autism spectrum and other neurodevelopmental disorders, to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, Clinical Care Managers consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. Job Description Position Summary: The Certified Peer Specialist (CPS)/Certified Recovery Specialist (CRS) focus on providing behavioral health intervention, and other recovery support services for adults with substance use disorder (SUD) and/or co-occurring disorders in the community as an integral part of the Mobile Outreach and Recovery Services (MORS) Continuity of Care (COC) team. The CPS/CRS Mobile play a crucial role in supporting individuals by promoting long-term recovery within the context of the community. CPS/CRS Mobile operates as a comprehensive support system, combining personal experiences, mentorship, advocacy, motivation, and respect for individuality. CPS/CRS aim is to prevent relapse, promote long-term recovery, and contribute to the development of a strong and supportive recovery-oriented community. The CPS/CRS work collaboratively with Behavioral Health Case Manager (BHCM) to provide support to individuals seeking behavioral health care and recovery support services in the community. CPS/CRS and BHCM professionals work ensure warm handoffs, smooth transitions, and continuity of care for individuals in need. As a part of the warm hand-off process, the CPS/CRS position involves facilitating linkages to care/services, providing support, conducting assessments, ensuring continuity of care, and maintaining accurate records to help individuals access services across a continuum of care to achieve their health and wellness goals in the community. The TIPS program will provide a comprehensive, intensive early intervention services for youth exhibiting truancy behaviors. The programming builds upon youths’ and families’ resiliency and sense of self-worth for positive outcomes and redirection, strongly networking with community resources. Assignments are based on a 40-hour week. Weekly schedules are developed based upon the caseload needs. There are no traditional office hours. Case managers will work primarily in the afternoon and evenings, specifically during the school year and at weekends to provide maximum involvement with the family and the identified clients. There may need to be a need to start early to help a participant get to school. The case manager will be placed on an on-call roster and will be responsible for providing emergency assistance to clients assigned in the program 24 hours a day while on duty. Case longevity is determined by court and or DHS personnel., • The case manager’s role is to be able to report to the court that identified issues have been resolved through a collaborative effort with school personnel and community resources., • Case manager must meet with the supervisor bi-weekly for supervision., • Case manager must meet with the family and see all children at least twice each month, and coordinate work with others involved with the family. DHS contact stipulations must be adhered to., • Case manager will form an “after hours on call” roster, with alternating back up of the program director. In this way, families can access our staff round-the-clock., • Case manager will ensure that services and counseling are commensurate with identified issues, and may need to be increased beyond minimum standards., • Following assessment of family’s identified needs, case manager will identify community resources, and work collaboratively with others to assist families with linking them to the appropriate community resources., • Case manager is responsible for ensuring that school age children receive the appropriate services that meet their educational needs., • Case manager will ensure that he/she keeps accurate documentation specifically all unsuccessful efforts to maintain the expected level of intensity and monitoring.
