Job DescriptionCase Manager PURPOSE OF POSITION The Case Manager is responsible for providing comprehensive case management services to individuals living with HIV/AIDS (PLWHA) who are engaged in supportive counseling and behavioral health services. This position ensures that clients receive coordinated, person-centered care that addresses their mental health, medical, social, and economic needs. The Case Manager works closely with clinical and support staff to assess client needs, develop service plans, coordinate care, and provide advocacy and linkage to essential services such as housing, financial assistance, substance use treatment, and healthcare.
