Job DescriptionDescription: DUTIES Provides skilled Physical Therapy services on an intermittent basis to consumers in their homes in accordance with physician orders RESPONSIBILITIES • Provides initial and ongoing comprehensive assessments of the consumer’s needs, • Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary, • Assists in development and implementation of the interdisciplinary consumer care plan as it pertains to Physical Therapy, • Assists the physician in evaluating consumers by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests, • Treats consumers to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light and electricity, • Consults and familiarizes staff regarding the established plan, • Teaches, supervises, and counsels the family and consumer in the physical therapy program, • Instructs consumer in care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices, • Provides the equipment required to carry out the plan, • Arranges for out-consumer service for procedures which cannot be given at home, • Records on a timely basis all evaluation data, treatments, and consumer's response to therapy intervention Also records quality assurance data, • Prepares an assessment of the consumer's condition and periodic evaluations and summaries, • Supervises Physical Therapy Assistants and paraprofessionals as per agency policy, • Records and reports to the physician the consumer's reaction to treatment and /or any changes in the consumer's condition, • Communicates with the physician per agency policy or whenever changes occur regarding the consumer’s status and obtains additional orders, if necessary, • Communicates with other team members (including Nurse Aides) and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the consumer, • Writes clinical/progress notes on the date of the visit and incorporates same in the consumer's record per agency policy, • Participates in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary, • Supervises the CNA per agency policy when physical therapy is the only professional discipline involved in the consumer's care and supervises the Physical Therapy Assistant per agency policy, • Obtains a medical history, particularly as it relates to the present condition, • Evaluates the consumer, the consumer’s significant other(s) and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the consumer, and what other Agency and community services will be required, • The ability to drive and the ability to access consumers’ homes which may not be routinely wheelchair accessible are required., • Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the consumer's condition and to perform consumer care., • On occasion, may be required to bend, stoop, reach and move consumer weight up to 250 pounds, lift and/or carry up to 30 pounds. Physical therapy equipment and other medical equipment will be utilized. Hand washing materials. COMPANY INFORMATION Has access to all consumer medical records which may be discussed with Nursing Supervisor, the Administrator, the Executive Director, and the Quality Assurance department staff Requirements: QUALIFICATIONS • Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association, • Must be licensed in Colorado, • Must have at least one year of institutional/home care experience, • Must have a criminal background check, • Must have current CPR certification
