Working in high-need Bronx neighborhoods, Peer Outreach Workers provide compassionate, low-threshold support and linkages to treatment, housing, mental health, and other essential resources. Description of Duties and Responsibilities. ▪ Engage PWUD and community members in parks, encampments, transit hubs, shelters, and other high-need areas. ▪ Distribute naloxone, fentanyl/xylazine test strips, sterile supplies, wound care kits, hygiene kits, and other harm reduction materials. ▪ Provide overdose prevention education and train individuals and families in naloxone administration. ▪ Offer peer support, motivational interviewing, and trauma-informed engagement strategies to build trust. ▪ Support clients with navigation of Medicaid, housing, benefits, and treatment referrals, including warm handoffs and accompaniment to appointments. ▪ Conduct field-based behavioral health screenings (e.g., PHQ-9, GAD-7) and refer clients to appropriate providers. ▪ Maintain accurate outreach records and ensure timely documentation of encounters, referrals, and outcomes. ▪ Collaborate with community partners to expand access to harm reduction and treatment services ▪ Participate in ongoing training, group supervision, and reflective practice sessions. Qualifications for Position: 1. Must have lived experience with substance use, recovery, and/or harm reduction., 2. Certified Recovery Peer Advocate (CRPA) credential is required or must be obtained within one year of hire., 3. Previous experience in street outreach, harm reduction services, or peer navigation is strongly preferred. Skills and Knowledge Required: Strong skills in trauma-informed care, harm reduction practices, overdose prevention, and motivational interviewing are essential, along with the ability to engage clients with empathy and respect. Job Description Job Title: Clinical Case Coordinator Job Summary: CASES is currently seeking a highly organized and detail-oriented Clinical Case Coordinator for our Pretrial Services Case Management Team. Rapidly after release into the program, the Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan. The Clinical Case Coordinator assists participants to comply with supervision appointments and court appearances during the pendency of their legal case. In this role, you will conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals. You will ensure participants receive services that are strengths-based, gender-specific, and culturally sensitive while adhering to the core principles of risk, need and responsivity to reduce recidivism. The goal is to increase engagement in community treatment services and support participants to achieve their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration. This position is based in our Bronx Community Office and fieldwork in the larger NYC community as needed to assist participants. SRP participants have not been found guilty of a crime and are not mandated to engage in treatment or other services. CASES’ Supervised Release Program (SRP) annually diverts 5,500 people otherwise facing pretrial detention at Rikers Island to community-based supervision and support services. Job Description Job Title: Case Coordinator Job Summary: CASES is currently seeking a Case Coordinator for our Brooklyn Pretrial Services team. Rapidly after release into the Supervised Release Program, the Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions required under the supervision plan. In this role, you will help participants comply with their supervision appointments and court appearances during their legal case's pendency. You will also conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals. You will ensure participants receive services that are strengths-based, gender-specific, and culturally sensitive while adhering to the core principles of risk, need and responsivity to reduce recidivism, increase engagement in community treatment services and support participants to achieve their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration. This position is based in our Brooklyn Community Office and fieldwork in the larger NYC community as needed to assist participants. CASES’ Supervised Release Program (SRP) annually diverts 7,500 people otherwise facing pretrial detention at Rikers Island to community-based supervision and support services. Program participants include youth and young adults, people with substance use disorders and mental illnesses, and individuals who are homeless. SRP participants have not been found guilty of a crime and are not mandated to engage in treatment or other services. This requires staff to tactfully deliver approaches to help participants tap into their intrinsic motivation to leverage their strengths and skills for meaningful life improvements while fulfilling all court requirements.
