This position is also responsible for assisting the Resident Care Director in day-to-day operations of the Memory Care Community. Expectations are to coordinate and implement programs and services designed for residents with cognitive impairment; meet or exceed standards of care; encourage and assist each Memory Care resident to function at their highest level of ability; and by word and action reassure families and friends that residents are safe, secure, and well cared for. Job Duties: Essential Functions: • Support the mission, vision, and culture of the organization through positive communication and leadership., • Contribute to Community relationships by demonstrating cooperation and professional conduct with residents, families, fellow associates, physicians, pharmacies and other vendors or persons., • Contribute to team effort by being flexible in work assignments; by furnishing support; by taking initiative; and by understanding how this position affects and compliments all other Community positions., • Able to maintain work pace appropriate to given workload. Perform activities on schedule while maintaining regular attendance and punctuality within specified tolerance., • Develop, maintain, and communicate accurate weekly and monthly staff schedules that align with budgeted labor hours and community needs., • Ensure all shifts are adequately covered while maintaining compliance with staffing ratios and regulatory requirements., • Monitor attendance, tardiness, and call-offs; adjust schedules proactively to minimize overtime and staffing shortages., • Coordinate shift coverage and approve schedule changes in collaboration with department heads., • Post schedules in a timely manner and communicate updates or changes promptly to staff., • Maintain scheduling data and updates in the time and attendance system (e.g., Homebase, OnShift) and ensure accuracy for payroll processing., • Track vacation, PTO, and leave requests to ensure appropriate coverage., • Review and approve employee shift swaps or coverage changes per company policy., • Forecast and adjust staffing levels based on occupancy, census, and acuity changes., • Partner with department leaders and HR to identify staffing trends, shortages, or overages and propose solutions., • Support recruitment efforts by communicating recurring staffing gaps or schedule challenges., • Maintain compliance with labor laws and company policies regarding breaks, overtime, and scheduling notice requirements., • Maintain clear records of all resident changes, requests, and irregularities in coordination with Assisted Living Director., • Update care plans in coordination and with direction with the Assisted Living Director., • Record and notify Assisted Living Director, Executive Director, physician, pharmacist, and family of any significant changes to resident’s health status and/or reactions to drugs, treatments, or significant incidents., • Assist as needed with order medications from pharmacy, maintain records, and ensure that medications are properly stored and cared for., • Audit med room and charts weekly., • Review and implement incident reports; evaluate for need of intervention to minimize risk of further occurrences. Escalate incidents to nurse for review., • Read and understand medications and charts., • Maintain records and files for residents, inventory, and supplies., • Maintain appropriate supplies at all time., • Provide quality assisted living and memory care program., • Monitor associates in administration of mediation, including frequent audits of medication records, • In coordination continually assess resident needs in relation to staff training needs to determine priorities in designing an ongoing orientation program., • Participate in New Hire Orientation program and other regular staff meetings or functions., • Report concerns and problems to supervisor., • Respond appropriately to changes in the work setting., • Be familiar with Carefield Living/ Carefield Senior Living Policies & Procedures Manual and Staff Handbook., • Perform other related assignments as required., • Attend required in-service training., • Be willing and able to work in any resident care position., • Conduct tours., • Pursue educational development in Alzheimer’s disease and related disorders., • Accountabilities:, • Act on constructive feedback by listening to supervisor(s), customers and peers and use it to improve performance., • Maintain resident, employee, and community confidentiality., • Respect Resident’s Rights., • Demonstrate positive attitude and ability to work well with all people, particularly the elderly., • Promote positive work environment that emphasizes teamwork., • Demonstrate willingness to help other staff whenever needed., • Observe universal precaution procedures and all safe workplace measures., • Act on constructive feedback by listening to supervisor(s), customers and peers and use it to improve performance., • Maintain resident, employee, and community confidentiality; abide HIPAA laws., • Respect Resident’s Rights., • Demonstrate positive attitude and ability to work well with all people, particularly the elderly., • Promote positive work environment that emphasizes teamwork., • Demonstrate willingness to help other staff whenever needed., • Observe standard precaution procedures and all safe workplace measures., • Report any suspected or observed abuse, neglect or misappropriation of property or funds of a resident to the appropriate licensing agencies., • This position also requires 2-3 years of supervisory experience, prior care experience in an assisted living community or skilled nursing facility, and valid First Aid and CPR Certification., • This position requires 2-3 years of experience with medication passing and management., • Able to speak, read, write and understand the English language fluently., • Maintain mental alertness, attention, and concentration for necessary periods., • Apply common sense understanding and carry out instructions (written, oral or diagrammatic)., • Adapt to situations requiring the precise attainment of set limits, tolerances, or standards., • Regularly required to sit and talk or listen., • Able to stand, stoop and bend frequently, lift up to 20 pounds and push up to 40 pounds. Also must have the ability to talk, hear, sit, use hands and fingers, handle/feel objects, tools or controls, and reach with hands and arms., • Specific vision requirements for this job include close vision, depth perception, and the ability to adjust the focus of the eyes., • Possess adequate physical stamina to move freely about the Community and assist residents where needed, including in emergency situations.
