Overview Provides a broad array of counseling/support services to individuals living with HIV/AIDS, and/or HIV negative, homeless and transgender persons, including persons with serious mental illness (SMI). Shares personal, practical experience, knowledge, and first hand insight to benefit program enrollees. Works under general supervision. What You Will Do • Acts as ‘Health Navigator’ and works with Members to develop and implement an individualized action plan. Homeless men are provided with comprehensive services to stabilize their condition in order to successfully transition into to permanent and/or supported housing. The Van Dyke facility will provide 54 units for New York/New York III formerly homeless families under NYC 15/15. The remaining 126 units will provide housing for low-income households, including a live-in superintendent. CAMBA’S Food Programs, made up of the Beyond Hunger Emergency Food Pantry and the NOEP Program, work to ensure that no one in Brooklyn goes hungry. The Beyond Hunger Emergency Food Pantry, in Partnership with a number of different government and private entities including the NYS Hunger Prevention and Nutrition Assistance Program (HPNAP), United Way of NYC (UWNYC), Food Bank of New York, City Harvest, and NYC Human Resource Administration (HRA), distributes a 3 day supply of 3 meals per day of nutritional food to CAMBA clients-as well as those referred to us by churches and other agencies – due to hunger or lack of funds for adequate food purchase. Services also include food stamp screening, application assistance and referrals for application assistance. Additionally, the program brings in a number of different providers to provide information and referrals for nutritional education and support, other benefits and services to help improve client’s health, maximize their household income and help them move toward greater self-sufficiency. The NOEP Program provides outreach to individuals and families who may be eligible for Food Stamps. Staff assists eligible persons in filling out their applications and provides referrals to other food and nutritional resources. Position: Pantry Assistant Reports To: Supervisor I Location: 2241 Church Avenue, Brooklyn, NY 11226 What The Pantry Assistant Does: • Assist in all aspects of the distribution of nutritional food packages to eligible clients, including, but not limited to, stocking shelves, assisting with deliveries, conducting intake and assessment, assisting clients in “shopping” at the pantry, etc., • Assist with management of stock room inventory., • Assist in food purchasing., • Assist with packaging food items., • Pick up food donations at a variety of locations., • Assist in all aspects of food drives., • Assist in all aspects of the produce link program (i.e., picking up and distributing vegetables)., • Assist in all aspects of the Homebound Pantry program., • Assist with resource development and community relations., • Assist with Food Stamp application process (i.e., Eligibility pre-screening to potential eligible, non-participating clients, follow up to ensure that eligible clients receive Food Stamps, etc)., • Provide food stamp counseling/advocate to remove barriers to receiving Food Stamps as appropriate., • Attend trainings/meetings as requested., • Assist with collecting and inputting all necessary data and provide all required information for weekly/monthly/quarterly/annual reports to CAMBA management and/or funders., • Help to make the necessary referrals to clients requesting particular services (i.e., job placement programs, WIC, HEAP, tax preparation assistance, childcare and other food and nutritional services)., • Light building maintenance i.e., fixing locks, and minor repairs., • May assist clients with access to tax preparation assistance via the Tax Preparation Initiative through the Food Bank of NY., • May facilitate Food Stamps education workshops (i.e., eligibility requirements, application process)., • May create outreach and marketing materials for the program., • Data entry experience., • Must be able to lift at least 25 pounds., • Bilingual Preferred., • Must have Valid New York State Driver’s License and excellent driving history., • Must complete NYS Approved Defensive Driving Classes every 3 years., • Must take Refresher Computerized Defensive Driver class., • Must remain insurable based on CAMBA’s point system throughout the duration of employment., • If required to drive 15 passenger van, must score at least 80% on computerized 15 passenger van improvement test every year., • If required to drive 15 passenger van, must pass an observed Van Road Test. Youth Enrichment Services (YES) engages school-aged youth between 12 and 21 years old in programming and supportive services that change the culture around violence and encourage positive youth development. YES is delivered in and around schools in neighborhoods with a high prevalence of community violence. YES staff actively recruit and engage youth who are vulnerable to system involvement and may be at risk for interpersonal violence or who have experienced community violence. Cure Violence is a public health strategy to reduce and prevent shootings and killings. This program will focus on a defined area identified as a “hot spot” in Brownsville, Brooklyn for shooting violence within the 73rd Precinct. The program works to prevent gun violence by: 1) mediating conflicts that may end in gun violence, 2) changing the thinking of individuals who are most at risk of being victims or perpetrators of violence, and 3) working closely with a broad coalition of local partners to promote a visible community-wide message that shooting is unacceptable behavior. Job Description Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports, they need to thrive. We take a comprehensive approach by offering more than 180 integrated programs in: Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach almost 80,000 individuals and families, including almost 13,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today. HomeBase- is a CAMBA program that provides case management and essential services to those at-risk individuals and families in Brooklyn and Staten Island to prevent homelessness. Individuals and families receive services that help them overcome problems with public benefits, housing, education, health care and other issues that could impede their ability to maintain stable households. Position: Program Supervisor Reports To: Program Manager Location: 209 Bay Street Staten Island 10301 What The Program Supervisor Does: • Communicate effectively with clients, direct reporting staff, peers, supervisors, and funders regarding best program practices., • Has input into and understanding of, Case Management aspects of contract requirements and communicates this understanding to staff., • Ensure that all client files and electronic files are kept in compliance with funder’s standards and implement CAMBA internal Quality Assurance protocol., • Analyze case management program and demographic data to make programmatic improvements., • Maintain all trackers, monitor scheduling/ staff calendars., • Review and ensure appropriate action is taken in response to program emails., • Supervise, coach, motivate and counsel direct reporting staff to excel., • Plan and organize program activities to maximize program contract's goals and performance targets., • Improve teamwork among direct reporting staff as well as among peers., • Troubleshoot and make decisions within the boundaries of authority level, in accordance with program policies, procedures, and protocols., • Work with direct reporting staff to improve work performance through client feedback, training, and other appropriate mechanisms., • Overcome resistance to change from clients, direct reporting staff, and supervisors/funders., • Administer constructive discipline to direct reporting staff, as needed., • Assist in recruiting of program eligible clients., • Confer with CAMBA’s Management staff as needed regarding programmatic and personnel issues., • Manage own time effectively and coordinate program activities to maximize time of direct reporting staff., • Monitor and review clients' progress weekly by conducting case conferences with staff., • Review all documentation related to clients' progress for accuracy, completeness, and clarity., • Participate in administrative and staff meetings as requested., • Provide all required information for weekly/monthly/quarterly/semester/annual reports to CAMBA management and/or to funders., • Prepare marketing materials for the program., • Reach out and market the program to the community in order to recruit clients., • Prescreen clients over the telephone for eligibility and may schedule intake appointments.
