SUMMARY Responsible for: • Facilitating referrals to all outside/specialty services., • Managing and update list of Manage Care members. Essential Duties, • Manages all patient referrals, ensuring appropriate follow-up and documentation, • Provides follow up for all referrals to L.A. County facilities (King/Drew Medical Center, Harbor-UCLA, LAC+USC, Olive View Medical Center), • Coordinates documentation and data entry for Cancer Detection Program mammography., • Manages and maintains database of high-risk patients and schedules follow-up care., • Assists clients/patients in scheduling appointments, transportation, etc., as necessary., • Communicates pertinent patient requested services that are not available in the clinic and offers appropriate referrals., • Maintains up-to-date clinical records and documentation to ensure safe and best practice., • Will be responsible for medical records and diagnostic imaging received as part of referrals process and subsequently closes out referral received., • Will be responsible for medical records correspondence within EMR and will have also be requesting appropriate medical records from previous referrals., • Manages and updates list of Managed Care members; mails welcome letters to new members., • Attends staff meetings as required, • Attends local community meetings related to expanding services available for UMMA patients; represents UMMA at various community events., • Performs a wide variety of clerical duties including: preparing routine correspondence,filing, photocopying, faxing, and answering the telephone; and referring patients to appropriate person., • Receives and treats all patients with the utmost respect and professionalism., • Adheres to strict standards regarding client confidentiality, informed consent, and disclosure as required by California law and agency policy., • Strictly adhere to universal precautions as established by the Center for Disease Control and Prevention, Occupational Safety and Health Administration, DHS/OA and the Clinic. Secondary Duties These specifications are general guidelines based on the minimum experience normally considered essential to the satisfactory performance of this job. Individual abilities may result in some deviation from these guidelines. • Successful completion and acceptable results of post offer physical examination, drug screening, and criminal background check. · High school diploma or GED equivalent · Prior experience in provider client services in a public health setting · Knowledge of fundamental health issues (diabetes, hypertension, asthma, etc) · Knowledge of social service structure and health care environment in LA County · Strong clerical/administrative skills including personal computer skills · Effective oral, listening and writing communications skills in both English and Spanish. · Ability to work effectively with people of diverse cultures, ages and economic backgrounds. · Current California driver’s license and legally required automobile insurance. Typically, these skills are the result of a combination of education and experience equivalent to a high-school degree or GED equivalent and several years of experience working in a community service organization preferably direct client services in a health related organization. The incumbent must be able to perform this job safely, without endangering the health or safety of him/herself or others.
