We are seeking an experienced and driven Registered Nurse (RN) to serve as the Administrator of our Licensed Home Care Services Agency (LCHSA). This is a 6-month contractual leadership opportunity with strong potential to transition into a permanent role based on performance and organizational needs. The Administrator will provide strategic and operational leadership to ensure the delivery of high-quality, compliant, and patient-centered home care services throughout New York State. Operational Leadership * Oversee day-to-day operations of the LCHSA, ensuring compliance with all applicable New York State Department of Health (NYSDOH) regulations. * Provide hands-on operational oversight, ensuring services are delivered efficiently, safely, and in alignment with regulatory standards. * Develop, implement, and monitor policies and procedures to maintain regulatory compliance and operational excellence. * Ensure readiness for audits, surveys, and regulatory reviews. Team Oversight & Development * Supervise and support a multidisciplinary team including: * Registered Nurses * Patient Care Coordinators * Home Health Aides * Provide leadership, mentorship, and performance management. * Foster a collaborative, accountable, and patient-focused culture. Regulatory Compliance * Maintain thorough knowledge of New York State home care regulations governing LCHSAs. * Ensure compliance with NYSDOH standards and reporting requirements. * Oversee proper documentation, patient records, and quality assurance programs. Home Care Registry & Systems Management * Manage and ensure proper utilization of the New York State Home Care Registry. * Ensure accurate and timely verification of home health aide certifications and compliance. * Oversee electronic health record systems and operational reporting tools. Quality & Patient Care Oversight * Monitor quality assurance and performance improvement initiatives. * Ensure patient care plans are appropriately developed, implemented, and evaluated. * Address patient complaints, incidents, and corrective actions as necessary. Required: * Active and unrestricted Registered Nurse (RN) license in New York State. * Minimum of 3–5 years of leadership experience in home care or a Licensed Home Care Services Agency (LCHSA). * Strong working knowledge of New York State home care regulations. * Hands-on experience using the New York State Home Care Registry. * Demonstrated operational management experience in a healthcare setting. * Strong leadership, organizational, and communication skills. Additional Information Heart and Vascular Cardiac ICU (5-1 SP) The Cardiac ICU is a fast paced, high acuity, 14-bed, critical care unit that cares for adult patients with complex cardiac, medical diagnoses such as myocardial infarction, cardiogenic shock, unstable arrhythmias, and heart failure. Our patients are treated with medical and interventional management. Nursing staff are trained in advanced heart failure management, including short- and long-term mechanical circulatory support (with devices such as intra-aortic balloon pumps, Impella catheters, and LVADs) and evaluation for advanced therapies and transplantation. RN-HVC Cardiothoracic ICU (CTICU/WP3) Cardiothoracic ICU is an 18-bed cardiac surgery unit, specializing in care for complex post-operative CABG, valve replacements, aortic dissection repairs, heart transplants and LVAD implantation. This is also the only unit in YNHH health system that cares for patients requiring ECMO, whether it is VV (lung bypass) or VA (heart and lung bypass). In addition, patients may require additional devices including IABP, Impella/VADs, and/or CRRT. Nurses will be trained to devices based on previous experience and progression of skills, expectations and critical thinking. HVC Cardiovascular ICU and Stepdown (5-4 SP) The Cardiovascular ICU and Stepdown is a 14-bed hybrid unit consisting of 8 ICU and 6 stepdown beds. We specialize in caring for patients post complex vascular surgery, such as aortic repairs, and post cardiac surgery patients such as CABG and valve replacements in our ICU beds. In the stepdown beds we care for patients with a variety of complex heart and vascular diagnoses.

Cardiovascular specialty care including medical and interventional management of coronary artery disease and atrial/ventricular dysrhythmias (cardia procedures such as catheterization, angioplasty/stent, pacemaker, ICD, ablations). HVC Cardiac Surgery Intermediate Care and Step Down Unit (5-3 SP): SP 5-3 is a fast-paced, dynamic 25-bed cardiac surgery unit, inclusive of 3 step down beds that cares for a diverse range of cardiac populations. This includes post-operative management for CABG, valve replacement and aneurysm repairs. HVC Cardiac Intermediate Care and Stepdown Unit (V3E): Verdi 3 East is a 30 bed cardiac unit located at the Saint Raphael Campus. Our unit's primary population include patients with cardiac procedures i.e., cardiac catheterization, angioplasty, pacemaker and defibrillators. The population also includes dysrhythmia management, heart failure and electrophysiology procedures. Six of our beds are designated for the cardiac step down population. HVC Cardiac Intermediate Care-Advanced Heart Failure and Stroke Unit (V5E): Verdi 5 East is a 29-bed cardiac unit located at saint Raphel Campus. Our unit patient population consists of advanced heart failure and stroke diagnoses including, advanced Heart Failure (HF), MI, Pre/Post Cardiac Catheterization and/or intervention, Pre/Post Pacer/ICD placement, Stroke patients post TNK after an ICU stay, those not eligible for TNK, ruling out for Stroke/TIA, and medical issues including COPD, Pneumonia, ACS, chest Pain and patients requiring inotropic or vasodilator support to enhance diuresis. HVC Cardiac Medical Intermediate Care Unit (5-7 EP): 5-7 is a 26 bed , fast paced , full telemetry heart and vascular unit. This is NOT a med-surg unit. Applicants for this position must be an OPWDD approved investigator with at least two years of experience conducting and completing Part 624 and Part 625 investigations. Essential Job Functions: • Investigates incidents and allegations of abuse as per Part 624 regulations and completes all necessary documents and follow up requirements., • Conduct investigations as needed regarding incidents relating to Corporate Compliance, Code of Conduct and HIPAA laws and regulations., • Supports the Incident Management Coordinator with reporting, documentation, and training for both internal and Justice Center investigations., • Assists to facilitate compliance and the timely maintenance of IRMA (Incident Report and Management Application) RIA (Restrictive Intervention Application) application within IRMA, and the Justice Center WSIR (Web Submission of Incident Report)., • Assists with inputting, maintaining and monitoring of incident reporting data management and documentation as per Parts 624 & 625 (in tandem with OPWDD IRMA, Justice Center, WISR online tools)., • Assists to facilitate the timely implementation of corrective action plans relating to incidents and allegations., • Gathers data for inputting and trending and participates in compliance monitoring, • Assists with preparing data for incident trends, reporting and follow up., • Assists with insuring appropriate documents are in place for ICC and HRC., • Must be an OPWDD approved investigator and able to conduct initial internal investigations for all incidents involving the Justice Center., • Facilitates that Opengate’s programs conform to Opengate’s policies and procedures., • Facilitates that Opengate’s programs follow federal and state governmental regulations and policies., • Performs targeted audits as directed., • Assists departments in the development of Plans of Corrections as directed., • Monitors that the interests of the residents are being pursued and that their rights are being protected., • Input & monitor investigatory updates/ uploads into the state incident management system and communicate them to the state agency., • Data compilation, review, analysis and reporting for routine and special projects/initiatives., • Participates on committees as assigned., • Demonstrates knowledge of and loyalty to the Opengate, Inc. mission, vision, and values., • Follows federal, state and local governmental regulatory guidelines pertaining to a safe, healthy and clean work environment, reporting health and safety concerns to the supervisor., • Identify and report any substantial Compliance Risks to the CCO or CEO., • Other duties as assigned. Qualifications: • High School diploma required, associate's degree in human or social services preferred., • Minimum of 2 years of compliance/quality improvement, utilization or program review., • Must be an OPWDD approved investigator with at least two years of experience conducting and completing Part 624 and Part 625 investigations., • Knowledge of state and federal regulations as they pertain to healthcare providers and operators of OPWDD services and the New York State Justice Center., • Minimum of 3 years of experience with OPWDD systems, including WSIR and IRMA., • Strong written and verbal communication skills., • Strong organizational and time management skills., • Computer skills and capacity to navigate and enhance proficiency in software systems as needed. Working knowledge of Microsoft Office and similar programs. Physical Requirements: This position operates in a professional office environment. The role routinely uses standard office equipment and requires the ability lift, push and pull in order to perform general office duties such as filing. This would require the ability to move files, open filing cabinets and bend or stand as necessary.
