The Primary Care Physician (PCP) in our organization demonstrates: • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors., • Coaching for health: The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients., • Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams towards their performance goals. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals. The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: • The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient., • The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient., • It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not)., • The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs., • The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office., • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market., • PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company., • Performs other duties as assigned and modified at manager’s discretion. Job Title: Primary Care Physician Location: Philadelphia, PA Compensation: Base salary starts at $250,000 with an incentive bonus up to $20,000 in the first year (and growing as a partner). The incumbent is responsible for the overall performance of the operational, economical, and people-related center level goals. They lead in providing overall direction to the operation and administrative aspects of the market, utilizing the related resources associated within the Centers. The incumbent will partner closely with the centers’ clinical leadership to ensure patient care, service and clinical metrics, goals and objectives meet and/or exceed expectations. You will not be expected to be able to perform any of the following job duties without first going through a training period. In addition, we are all always learning from each other and from the dogs, and experiencing daycare first-hand is the best way to learn dog behavior and to become comfortable with acceptable types of play. You will never be asked or expected to do anything for which you are not fully trained and comfortable. As a part of our team here at Doggie VIP, you will be responsible for supervising play in our busy doggie daycare service. Much of the job involves monitoring play while being aware of various play styles and ensuring that all dogs are safe, stress-free, and having fun. You will also be expected to help maintain a clean play space for the dogs by watching for, picking up, and properly disposing of any waste. You will also help dispense meals for the dogs during your shift, and may be asked to administer medications. We are a company that empowers team work and therefore everyone is expected to help with other tasks when needed and support team members. Position Summary: The Director of Network School Operations is responsible for overseeing the school operations program across multiple Mastery Schools. Reporting to the Senior Director of Network Operations, this individual serves as a partner and strategic resource, supporting school-based operations teams and driving effective implementation of the Mastery Operations model. The Network Director coaches Assistant Principals of Operations (APOs), collaborates with central office teams, and ensures operational systems are well-designed, consistently executed, and continuously improved. Duties and Responsibilities: • Drive student achievement and set high expectations for all students, • Commit to professional growth, self-reflection, a receptiveness to feedback, and a desire to continuously improve, • Implement rigorous and appropriate lesson plans, assignments, and assessments in cooperation with Mastery school-based leadership and curricular resources developed by Mastery’s central office Academic Team, • Work closely with school leaders to analyze student assessment data to measure progress and use data to inform instruction, • Collaborate in grade level teams to discuss student work, share best practices, plan events for joy and humor, and ensure student mastery of standards, • Engage families in their children’s education by building relationships and maintaining regular communication, • Demonstrate genuine interest, belief, and care for students’ personal and academic success, • Respond positively and effectively to challenges with a solutions-oriented resiliencyEducation, Experience, and Skills:, • Bachelor’s degree required with a record of personal, professional, and/or academic achievement, • Teacher certification - completed or in process, • Demonstrated expertise in subject area, • Outstanding instructional skills driven by data and delivered through rigorous and engaging strategiesPhysical Requirements:, • Ability to physically perform the essential duties of the role, and to work in the environmental conditions required, such as: traveling to network campuses; maneuvering in office spaces (including standing, walking, sitting for long periods of time, speaking loudly and clearly, seeing and hearing things both near and far away); stooping, kneeling, reaching file cabinets/shelves; fine finger and hand manipulation in use of computer, chalkboard, dry erase, &/or projectors; filing, faxing, scanning, coping, typing, mailing, and making phone calls; sitting for up to two (2) hours looking at a computer monitor, using a keyboard/mouse, and typing.Salary and Bonus Information:, • This role is eligible for signing and relocation bonuses.
