Job Description & Requirements • Specialty: Med Surg, • Discipline: RN, • Start Date: ASAP, • Duration: 13 weeks, • 36 hours per week, • Shift: 12 hours, • Employment Type: Local Contract The Registered Nurse (RN) provides comprehensive nursing services across the Crisis Respite/Transitional Program (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI) needs by integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN collaborates with interdisciplinary teams to promote health, safety, dignity, and successful community integration. The RN ensures that complex medical, behavioral, and psychiatric needs are met during the transition from an inpatient setting to the community. ESSENTIAL RESPONSIBILITIES (including but are not limited to): Clinical Care \t • Screen individuals who are being considered for admission to the CRTP and the Transitional Respite Program. Active member of the "intake” team.\t, • Conduct initial and ongoing nursing assessments for individuals admitted to CRTP.\t, • Oversight of self-medication administration program, monitor therapeutic response, and document outcomes.\t, • Provide direct nursing interventions such as wound care, diabetes management, respiratory illness support, and chronic disease oversight.\t, • Monitor early signs of medical or psychiatric crisis and initiate appropriate interventions.\t, • Regularly reviews medication administration records, logs documenting vital signs, elimination, glucose readings, weight, menses, and other records as individually specified.\t, • Reports significant issues to appropriate clinicians and team members, including the need for ordering medications, renewing or updating prescription orders, and people-supported specific drug information sheets. \t, • Monitor and manage medication storage and disposal procedures in accordance with federal and state law.\t, • Develop and monitor health goals that are person specific. Review, update, and revise these documents to ensure they are reflective of the current person supported status.\t, • In compliance with OSHA mandates, monitors infection control, environmental changes, and ensures that all guidelines regarding universal precautions are appropriately followed. \t, • Order and maintain necessary first aid and medical supplies as needed.\t, • Partner with ICM teams to address both medical and behavioral health needs in service planning.\t, • Deliver health education and coaching to individuals, families, and direct support staff to promote independence and wellness.\t, • Facilitate hospital discharge and community reintegration through CTI, ensuring continuity of medical, psychiatric, and social support.\t, • Serve as the clinical liaison among providers, families, and program staff.\t, • Participate in interdisciplinary treatment planning and case conferences.\t, • Train and mentor direct support professionals (DSPs) on health-related tasks and emergency response.\t, • Current New York State RN license (unrestricted).\t, • Associates Degree \t, • Minimum 2 years of nursing experience; background in IDD, behavioral health, crisis care, or community health preferred.\t, • Familiarity with OPWDD, OMH, and DOH nursing regulations and program requirements.\t, • Strong clinical assessment, crisis intervention, and care coordination skills.\t, • Excellent communication, organizational, and documentation skills.\t, • Ability to work independently while thriving in a team-based environment.\t How you project yourself in the world says a lot about you.
