Description Salary 35235 - 39513 per annum Pro Rata Permanent Part Time 18.5 hours per week OP&PD Chorley Lancashire International Sponsorship - this role is not open to international sponsorship We are currently recruiting Social workers with empathy and the drive to make a difference finding creative ways to support people to achieve what they want to and live as independently as possible in a place they call their home. We need a permanent part-time Social Worker to work across Lancashire as part of our Adult Services Older Adults and Physical Disability Community Teams. As part of your exciting new role you will be expected to undertake strength based outcome focussed assessments and support planning ensuring that the residents of Lancashire receive the right amount of support. You will play a vital role in supporting people to access local services maintain independence promote health and wellbeing and to live independently. A key part of the role includes working alongside other agencies professionals and informal carers providing a holistic service.
