This is an exciting opportunity to play a vital role in supporting patients to transition safely and effectively from hospital to home, ensuring care is delivered in the right place at the right time. As part of the multidisciplinary team, you will provide timely assessments, interventions, and recommendations that promote independence, prevent unnecessary admissions, and support patients in achieving their goals in the most appropriate environment. You don't need experience in care to start your career with us; we recruit on values and transferrable skills.
