The RN Clinical Care Coordinator will be the primary care manager for a panel of members with complex medical/behavioral needs. Care coordination activities will focus on supporting member's medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care. This position is a field-based position with a home-based office. You will work from home when not in the field. Local travel up to 75% and mileage is reimbursed at current government rate. If you reside in the Fall River/New Bedford, MA area, you will have the flexibility to work telecommute* as you take on some tough challenges. Pre-employment Education/Experience: • Masters degree in counseling, social work, clinical psychology, or closely related discipline required, professional license desired., • Five years or more years of administrative and management experience coupled with a Bachelors degree in a Human Service related discipline can be substituted for the Masters degree, • Knowledge and practice of policies, regulations, and standards, • Knowledgeable about self-help groups and peer counseling, • Maintains good communication with other programs that our consumers may be receiving services from
