Job Description Reports to: Shelter Director Status: Full-time FLSA Classification: Exempt Job Location: Ozone Park, Queens Salary Range: $75,000 - $85,000 We are seeking an experienced and progressive Director of Social Services for our men's mental health shelter. This position oversees the Clinical and Social Services functions to optimize the quality of care to residents in accordance with DHS regulatory and accreditation requirements. The ideal candidate will develop and supervise two Social Service teams, each consisting of Social Workers, Case managers, Housing Specialists, and Peer Engagement Specialists. Complete mental health assessments of shelter clients if necessary., • Develop assessments, counseling methods, programming, and referral resources to ensure that clients coping with substance and alcohol use issues are fully supported in attaining harm reduction goals., • Provide information and resources to assist clients in toward achieving housing goals., • Work with clients and case managers in identifying community-based referrals for a client to continue treatment after leaving the shelter., • Develop and implement crisis prevention and/or intervention strategies to ensure a safe and secure environment., • Provide assistance and follow-up in regards to critical incidents with clients including the development of safety plans., • Ensure shelter adheres to a Good Neighbor policy by working with social service staff to develop a guide of community resources where clients can appropriately spend leisure time during the day. Required Education and Experience: • Master's degree in social work (MSW), Mental Health Counseling (MHC) or Marriage and Family Therapy (MFT) from an accredited school. Identify, plan and arrange for appropriate services by applying a knowledge of local regional and federal services and regulations. Collaborate with the participants, programming teams and community services., 6. Coordinate implementation of the care plan with case conferences and working collaboratively with community based partners on these conferences. to maximize continuity of care, 7. Monitor the participant’s needs and responsiveness to care plan interventions. Communicate with providers who are delivering care and services for the purpose of maintaining the quality of care. Maintain and revise care plan as the participant’s needs change., 8. Document assessments, plans, interventions, the participant’s goals and their response to care management. Evaluate the participant’s response to the plan of care and the appropriateness of services. Collaborate with team members to identify the participant status and need for care plan adaptation., 9. Participate in process improvement groups. Utilize benchmarking and other sources to identify best practice. Participate in setting performance targets to improve quality. Promote and assist with development of appropriate education materials. Promote and facilitate effective communication throughout the continuum of care., 10. Maintain, update and actively improve care management skills, including identification of high risk individuals, care coordination, transition planning and implementation and knowledge of community resources. Work collaboratively with SkillSpring staff., 11. In collaboration with the Workforce team, facilitation of young adult programming including professional development, clinical skills training, and health career exploration., 12. In collaboration with the Workforce team, monitor participation of young adult programming by scheduling, tracking and reporting on various data points, including but not limited to attendance and surveys., 13. In collaboration with the Workforce team, support the SkillSpring team, leading and supporting other program activities as needed., 14. Create an environment that is diverse, culturally sensitive and relevant to the needs of young adults., 15. Perform other related duties as required. *This position may require some mornings, evenings and Saturdays at multiple sites. Hours, days and locations fluctuate based on seasonal changes. Skills & Requirements JOB QUALIFICATIONS: • Bachelor’s Degree in Social Work required, • Experience with underserved, urban young adults and first-generation college students, • Passion for working with youth and young adults, • Familiarity with healthcare career trajectories, • Experience developing new roles or new initiatives, • Excellent interpersonal skills; Engaging, extroverted personality is a strong asset, • Strong computer literacy- specifically Excel, is required, • Ability to read, analyze and interpret raw data, • Working knowledge of a CRM is highly desirable, • Ability to write reports, business correspondence, and procedure manuals (writing sample required), • Experience in planning and facilitating group workshops/events, • Ability to work independently and as a part of a team, • Excellent verbal and written communication required, • Attention to detail and deadlines, • Flexibility and adaptability a must, • Bilingual preferred (Spanish/English) PHYSICAL DEMANDS: • Required to stand and talk or hear, walk and sit., • Lift and/or move up to 25 pounds., • Travel to multiple divisions when necessary.
