POSITION OVERVIEW Provide medical care and services for clients with history of chemical dependence, including health history, physical examination, minor office procedures, treatment, referral, counseling and follow up care and services. KEY ESSENTIAL FUNCTIONS • Under the supervision of the Nurse Supervisor or Direct of Nursing, act as the health coordinator for the programs to provide services to the clients in the program., • Review potential client medical histories submitted by the intake department., • Assist with referring clients to appropriate health care provider specialists., • Provide oversight of the LPN and CNA staff, • Conduct medical assessments with all new clients., • Assist with the managing client prescriptions with various pharmacy, focusing on the agency preferred pharmacy., • Coordinates with the client’s primary care physicians to inform decisions regarding management and treatment of problems identified and provides care as prescribed by physician., • Manage client care: follow up plan and monitor progress, determine when referral to another provider is necessary and plan for further care, determine when emergency room visit is necessary and when illness is resolved., • Work with interdisciplinary team members regarding modification of treatment as necessary according to health concerns., • Provide oversight and monitoring of the wellness center to promote organized and efficient medication tracking, refills, and disposal of medication, as needed., • Document all medications in electronic health record system (e.g. NextGen, AWARDS)., • May participate in staff meeting or professional development activities as needed., • Assist in staffing shortages., • Bachelor’s degree in nursing preferred from an accredited nursing program., • Must possess an active and unrestricted license as a Registered Professional Nurse (RN) in New York State., • Minimum of two (2) years of related experience in health care facility., • Must possess current Basic Life Support (BLS) certification., • Must possess current NYS approved Infection Control certification (4.0 hours, valid for 4 years)., • Must have specific training in chemical use, abuse and dependence specific to the services provided., • Must obtain Mandated reporter (2 hours) prior to hire date., • Obtain within six months of hire:, • Supporting Recovery with Medications Addiction Treatment (MAT), • CASAC Canon of Ethics (6 hours), • Confidentiality related to 42CFR (3 hours), • Knowledge of substance/alcohol dependence/abuse population. Position Overview The Certified Recovery Peer Advocate requires someone who is culturally aware and sensitive to the needs of the targeted population, possess excellent computer skills, be able to function as a part of an inter-disciplinary team and have understanding of therapy and group work rooted in evidenced based approaches for intervention and management strategies. Certified Recovery Peer Advocate services are peer-delivered services with a rehabilitation and recovery focus. They are designed to promote skills for coping with and managing behavioral health symptoms while facilitating the utilization of natural resources and the enhancement of recovery-oriented principles (e.g. hope and self-efficacy, and community living skills). The Certified Recovery Peer Advocate uses trauma-informed, non-clinical assistance to achieve long-term recovery from a behavioral health disorder. Under supervision of the Program Director, the requirements listed below represent the knowledge, skill, and/or ability required. The provider ensures strengths based, person centered quality of care provided in a culturally and linguistically meaningful therapeutic environment. POSITION OVERVIEW Under supervision of the Program Director the Clinical Supervisor will provide clinical oversight and quality assurance to all unlicensed and licensed staff clinicians who provide substance and alcohol abuse services in an OTP program setting. The Clinical Supervisor is responsible for adherence to all applicable licensing, supervision, billing, documentation requirements and overseeing the timeliness and quality of a staff member’s clinical work. The Clinical Supervisor is responsible for client case record reviews, performance improvement and quality improvement to include all required monthly reports. KEY ESSENTIAL FUNCTIONS • Responsible for the daily operations and performance of the OTP program, • Manage OTP clinic operations to budgeted and planned results., • Work closely with staff via weekly supervision to ensure the completion of performance goals. Provides ongoing, regular feedback and performance evaluation to clinicians on the quality and timeliness of their clinical work as well as their caseload. Also solicits feedback from clinicians regarding supervision and whether needs are being met., • Monitor caseloads and ensure the supervisees comply with all agency, clinical, legal and ethical practices. Review and co-sign supervisees’ clinical documentation to include but not limited to methadone dose change request forms, bio-psychosocial history, initial treatment plans and treatment plans reviews., • Becomes familiar with APG and ICD-10 billing codes to include DSM V diagnosis codes., • Oversight and monitoring of billable hours of service required. Provide support, training, and remediation where needed to assist supervisees in meeting program’s billing expectations and caseload requirements, charting, recordkeeping, documentation standards and all other quality assurance standards., • Communicate any concerns about quality of clinician’s work to Program Director in a timely manner., • Become familiar with ACACIA policies and procedures and implements in OTP program. Ensures all administrative and clinical policies and procedures are implemented appropriately and effectively. Also ensures all medical and other services to which patients may be referred are known to staff and made available to patients., • Communicate with and ensures compliance with regulatory bodies (OASAS, Joint Commission, SAMHSA, DEA, HIPPA) to include state and federal regulations., • Assist with partnering opportunities, linkage/support agreements and other critical relationships to drive census growth and meet regulatory requirements., • Work closely with Medical Director, Program Director, and Operations Director in a variety of areas including but not limited to: compliance with federal and state rules; laws and regulations; compliance with Joint Commission, SAMHSA, OASAS and DEA; performance improvement initiatives; measure of success; productivity goals; quality client care; customer service; operational enhancement; growth of client census; contractual requirements; standardization and conformity; best practices and plans for continuous improvement, • Become familiar with Joint Commission and OTP federal and state laws, regulations, etc. and the applications of such., • Participate in community relations and development activities to drive and maintain census., • Identify and implement tactical steps to increase and retain census., • Work with clinic team to ensure operations are prepared to handle increased census., • Identify training needs and makes recommendations to the Program Director. Provides education and training opportunities to all staff to meet required training needs for OTP program., • Participate collaboratively as part of a multidisciplinary team, able to proactively collaborate with other supervisors and program director to ensure best possible training for supervisees and service delivery for clients., • Facilitate weekly case conferences and group supervision; monthly clinical and medical staff meetings to include incident reporting, client satisfaction surveys, PI, QI and chart review findings., • Participate in Acacia meetings, program meetings and required trainings as required., • Contribute to a six month and annual performance evaluations of clinical staff. The Nursing Supervisor Coordinates facility operations in the absence of facility leadership. Follows all infection control protocols. Follows attendance, dress code, and scheduling guidelines. (9am-5pm M-F). POSITION OVERVIEW Under the direction of the Senior Administrator of Population Health, the Health IT Manager is responsible for aligning the technology development strategies with emerging needs of La Casa de Salud’s primary care operations and value-based infrastructure. Position Summary: The Assistant teacher is responsible for supporting a learning environment based on the Creative Curriculum approach that helps the children develop physically, emotionally, socially, cognitively and creatively. They are responsible for complying with procedural norms, standards of attendance, and daycare regulations . Supervised by the Director of Early Learning Center, the assistant teacher helps make the center a warm, welcoming environment. Responsibilities include: ● Ensure a learning environment based on the Creative Curriculum approach that helps the children develop physically, emotionally, socially, cognitively and creatively. ● Support the implementation of activities that will promote social, emotional, cognitive, physical and creative development of children. ● Adhere to all Head Start Performance standards in addition to the Agency mandates of United Community Day Care center. ● Assume responsibility for the operation of the classroom, in the absence of the Group Teacher, thus holding the teacher aide and substitute accountable to their duties and responsibilities. ● Share job related knowledge with team members so as to promote and support their professional growth. ● Participate in regularly scheduled staff & team meetings, classroom development workshops on and off site as well as training conducted by UCDCC, ACS and other funders. ● performs center- related services/activities such as parent/teacher conferences, transition meetings, end of year celebrations, etc. ● Share information regarding children and the program with parents via daily contact and informal/formal interfacings, newsletters, etc. ● Encourage parents to visit classrooms to observe as well as volunteer during the course of the day. ● Work closely with parents to foster mutual respect on all levels. ● Encourage parents in their role as their children’s primary teachers by acting as resource person (i.e.: disseminating information, actively listening to parents, offering suggestions, etc.). ● Establish and maintain positive relationships with parents/families via open communications, training and other developmental activities ● Elicit input from teacher aide to complete written observations, progress reports, outcomes reporting, CROWD and other documents relevant to the tracking of children’s progress. ● Assist Group Teacher utilize information obtained via observations, conferences, tracking, etc. to design developmentally appropriate and individualized lesson plans. ● Adhere to Agency (UCDCC) policies and procedures; seeks guidance/clarification from Educational Director re: interpretation, applicability and implementation. ● Strictly adhere to, and enforce, the Agency policy regarding Confidentiality and HIPAA requirements. ● Other related duties as assigned, consistent with the above. Qualifications: ● 30 credits, 60 credits, BA in ECE or CDA ● The assistant teacher shall meet the requirements set by the Agency for NYC Department of Education. ● Computer proficiency. ● Familiarization with child development and parenting techniques. ● Must clear DOE fingerprinting and background authorizations. ● Ability to demonstrate proactive leadership, initiative, positive problem solving, decision-making, and work in a team context ● Ability to communicate in a positive manner with management, staff & parents. ● Maintain confidentiality. ● Willingness to follow protocol, policies and chain of command. ● Ability to observe behaviors and record objectively. ● Open-minded, flexible, and willing to experiment (creative) ● Physical Requirements - Frequent sitting, standing, walking, talking, hearing, seeing, stooping, kneeling, crouching, & crawling.
