Job DescriptionRegional Grant Coordinators Be part of a transformative effort to improve the health and well-being of rural communities across Indiana. Our client is leading Indiana’s 5-year Rural Health Transformation Program (RHTP), including the Make Rural Indiana Healthy Again (MRIHA) Regional Grants, a historic $600M investment to strengthen healthcare access, prevention, and system resilience across 64 fully rural and nine partially rural counties. This initiative brings together state leadership, healthcare providers, and community organizations to design and implement solutions that reflect local needs while advancing a shared, statewide vision for better health outcomes. This is a high-visibility, field-based role with direct impact on healthcare access and outcomes across your region. About the Role As a Regional Grant Coordinator, you will serve as the operational backbone of this initiative at the regional level, helping turn statewide strategy into on-the-ground action. You will manage grant implementation, support regional partners, and ensure alignment with state and federal requirements. In this role, you will act as the primary liaison between state agencies, regional coalitions, and statewide partners, guiding communities through planning, execution, and reporting efforts that expand access to care and improve health outcomes. What You’ll DoGrant & Contract Management • Serve as the primary regional contract manager for organizations receiving RHTP funding, • Monitor compliance with state and federal requirements, including budgets, allowable expenses, and documentation, • Ensure regional coalitions meet milestones, deliverables, and timelines, • Identify risks or performance issues and escalate as needed, • Maintain audit-ready documentation using state-approved tools and systemsPartner Coordination, • Collaborate with statewide Technical Assistance Providers (IHA, IPHCA, IRHA), • Participate in regional meetings, workgroups, and planning sessions, • Align regional strategies with CMS and state priorities, • Share insights and best practices across regionsReporting & Performance Tracking, • Coordinate data collection and performance tracking aligned with CMS requirements, • Ensure timely submission of regional reports to IDOH, • Support partners in meeting reporting expectations and documentation standards, • Monitor outcomes and support corrective action planningStakeholder Engagement, • Serve as the day-to-day liaison between IDOH and regional partners, including hospitals, clinics, EMS providers, and community organizations, • Ensure regional efforts reflect local needs and priorities, • Maintain strong communication with statewide leadership and teamsProgram Implementation, • Help partners interpret program guidance and funding requirements, • Track progress, identify barriers, and support problem-solving, • Promote consistency through shared tools, templates, and frameworks What You BringSkillRequired / DesiredYears of ExperienceBachelor’s degree in public health, health administration, public policy, business administration, or related fieldRequiredExperience in grant management, contract oversight, program administration, or related workRequired5 yearsExperience working with healthcare organizations, rural communities, or public health systemsRequired3 yearsStrong understanding of compliance, budgeting, and reporting requirements for state or federal grantsRequired2 yearsAbility to manage multiple projects simultaneously with strong attention to detailRequiredExcellent communication, facilitation, and relationship-building skillsRequiredAbility to work independently in a remote, field-based environmentRequiredMust reside within the assigned RHTP region and be willing to travel regularly within the regionRequiredExperience with CMS-funded programs or federal cooperative agreementsHighly DesiredFamiliarity with Indiana’s rural health landscape, including hospitals, FQHCs, EMS, and community-based organizationsHighly DesiredExperience collaborating with statewide associations or multi-stakeholder coalitionsHighly DesiredKnowledge of rural health delivery, care coordination, or health system transformationHighly DesiredWork Environment & Travel • Field-based / remote — must reside within assigned region, • Regular travel within assigned region required, • Region 1: Elkhart, Fulton, Jasper, Marshall, Newton, Pulaski, Starke, • Region 2: LaGrange, Steuben, DeKalb, Noble, Kosciusko, Cass, Miami, Wabash, Huntington, Whitley, Wells, Adams, • Region 3: Benton, White, Carroll, Clinton, Warren, Fountain, Montgomery, Boone, • Region 4: Grant, Blackford, Jay, Tipton, Madison, Randolph, Henry, Wayne, Fayette, Union, • Region 5: Vermillion, Parke, Putnam, Clay, Owen, Sullivan, Greene, • Region 6: Shelby, Rush, Decatur, Franklin, Jennings, Ripley, Dearborn, Scott, Jefferson, Switzerland, Ohio, • Region 7: Brown, Lawrence, Jackson, Orange, Washington, Crawford, Perry, Harrison
