Job Description Job Title: Case Manager Department: Community Services FLSA Status: Permanent Job Status: Exempt Reports To: Community Services Director Supervises: None Position Description: Working within the Community Services Department, the Case Manager will conduct client intake, assess client eligibility for emergency financial assistance, develop self-sufficiency plans, housing stabilization plans, and connect clients to resources and services. The Case Manager will establish a client relationship that will serve to motivate and engage the client in his or her own program outcome. Essential Functions: Case Management • Recruit, screen, enroll, and work with clients who qualify for Wesley programs., • Complete initial program assessment to determine program/service eligibility and conduct client intake interviews to determine eligibility for services., • Provide one-on-one-case management services., • Advocate on behalf of clients’ access to services., • Screen potential clients for other Wesley programs; ensure warm hand-off to Wesley or other wrap-around support services (food pantry, emergency financial assistance, childcare, transportation, etc.), • Maintain appropriate confidential client file data and produce monthly summaries of activities., • Maintain client records, collect data, conduct follow up telephone calls and appointments and facilitate, as necessary, services through outside organizations and scheduling as necessary for clients., • Develop and implement individualized Housing Stabilization Plans to increase income, reduce expenses, and identify client-specific goals., • Coordinate inspections and document compliance with Housing Quality Standards (HQS), • Track and be accountable toward grant goals; regularly update agency databases and ensure accuracy of data being collected; assist director to fulfill reporting requirements as designated by department and funders., • Maintain and update HMIS for Wesley case management programs, • Review all client files for accuracy per funding sources, • Accurately document all case management activities, assessments, and progress notes in HMIS, Apricot, and other required systems., • Maintain timely and accurate records in accordance with program, funder, and regulatory requirements., • Submit all required documentation for financial assistance payments and ensure timely disbursements to landlords, rental agencies, or utility companies. Job Description Job Description Job Title: Night Auditor The Night Auditor responsible for the overall operations and appearance of the front desk of a hotel, typically during the overnight shift. The Night Auditor is also responsible for attending to all front office needs, the needs of guests, especially during check-in and check-out and is responsible for balancing the revenue and expense transactions, which occurred during the day at the hotel. This essential position is also responsible for reporting of revenues and activity of the hotel to regional and corporate level systems as directed. Job DescriptionDescription: Purpose: To provide a structured approach for community health workers to support participants engaging in RBPM in the Pressure Check Study, helping them to adhere to self-monitoring and attendance to blood pressure visits. In addition, CHWs will identify, assess, and refer and follow-up when indicated for participants facing social barriers such as: transportation, medical access, utilities, technology access and literacy, food insecurity, and housing instability. Role of a Community Health Worker (specialized in hypertension) CHWs are responsible for: • Reinforcing BP monitoring practices at home., • Supporting patients with appointment reminders and telehealth set-up., • Participate in a weekly huddle with the team of health advocates, clinicians, and supervisor. Review the week’s priorities, discuss any urgent needs, and align team goals., • Share any updates from previous days, including unresolved issues or successes., • Review reports in RedCap to make sure data entry is complete and correct, • Make reminder calls to patients scheduled for upcoming Pressure Check medical appointments, • Make reminder calls before Pressure Check Study Visits, • Transportation: Any barriers with transportation (if in-person visit), • Appointment: Remind patient of (date, time, location) and confirm that the patient is prepared for their appointment and address any last-minute concerns., • Technology: appointment inquire about connectivity issues or concerns with schedules., • Document all communications in RedCap using the Contact Log and schedule any necessary follow up call., • During reminder calls or follow up calls for RBPM appts, check on any outstanding SDOH needs., • Conduct a formal check-in at 1 month to make sure the participant’s Stel Hub is working properly, they are regularly measuring their blood pressure and that priority 1 SDOH need has started to be addressed., • At 3 months, ask the patient if the resources provided have been helpful and if there have been any outcomes., • At 6 months, contact the participant. Notify them of their graduation from the study and close out any resolved needs and document updates in Redcap., • Loosely follow same structure of RBPM follow-up, have biweekly check-ins during the first 3 months of their study participation, reduce to once-a-month check-ins until 6 months of enrollment has been completed., • After 3 no contacts/no shows to appointments or phone calls, attempts will no longer be made., • If the clinical team also cannot reach participant after 3 no contacts/no shows, please discontinue EPIC/Stel connection., • Study staff should still attempt to reach participant for 6-month follow-up appt. o Conduct follow-up calls 48 hours after every participant’s clinical appointment (business days)., • Purpose: Address any clinical questions the patient may have, such as those about care plan and or prescriptions § Communicate any clinical questions back to the clinical team for further information., • Track all patient communications, including the timing of appointments in RedCap RBPM appt log., • Update the status of follow ups, appointments, and SDOH needs in SDOH tracker., • Review our day’s work, ensuring all tasks have been documented and follow ups are scheduled., • Communicate any unresolved issues or resource needs to your supervisor o Prepare for the next day by reviewing upcoming tasks and scheduling any necessary calls., • Contact the participant 1 week before the 6-month follow-up appt and remind them as they approach the end of this period. Discuss any ongoing SDOH needs and plan prior to end of intervention arm.Requirements:
