How You’ll Impact Care: We’re seeking a compassionate and skilled Mobile Nurse Practitioner to join our Wound Care Team. In this role, you'll deliver advanced wound care in patient homes, skilled nursing facilities, and other clinical settings across your region. You will assess, diagnose, and treat complex wound conditions using evidence-based techniques to promote healing. Clinical Care & Wound Therapy · Be an educator: Empower patients, families, and partners with knowledge that supports wound healing. · Center and guide: Conduct holistic assessments as the foundation for thoughtful, patient-first care. · Offer diagnostic stewardship: Examine, diagnose, and treat using evidence-based, technology-forward methods. · Apply clinical expertise: Perform advanced wound care procedures with skill and precision. · Translate data into direction: Order and interpret labs, imaging, and diagnostics. · Guide the healing journey: Monitor progress and adapt care plans as needed. · Listen deeply: Understand patient concerns and deliver empathetic, whole-person care. Collaboration & Coordination • Work in trusted partnership: Collaborate across disciplines to ensure seamless care., • Shape the standards of care: Contribute to policy and protocol development., • Chart the journey: Ensure accurate, thorough EMR documentation for every encounter. Mobile Care Delivery: • Mobile Care Delivery • Extend care across settings: Travel to homes, SNFs, and other field locations., • Navigate with intention: Manage travel logistics while maintaining professionalism., • Drive responsibly, serve reliably: Maintain a clean driving record and adhere to mobile safety protocols. What You Bring • Board Certification as a Nurse Practitioner (NP) or Physician Assistant (PA-C), • Master's in Nursing (MSN) preferred, • Wound Care Certification (WCC, CWCN, CWON, CWS, CWOCN) preferred, • Bilingual English/Spanish is preferred (verbal & written), • 3+ years of NP experience preferred, • 3–5 years of wound care experience preferred, • CPR certification, • Valid driver's license and clean driving record Disclaimer: JAE Consulting Inc is an Equal Opportunity Employer.
