We are currently seeking a Registered Nurse to work at our Wernick Adult Day Health Care Center as the Clinical Coordinator. Position Summary: To ensure that preventative measures, treatments, medications, nutritional care, rehabilitation services, and related services are carried out, recorded and reviewed, in accordance with participant assessments and plans of care. Benefits: • Free weekly groceries, • Health, Dental and Vision Insurance, • Basic Life Insurance, • Voluntary Insurance (Accident, LTD, STD, Whole Life Insurance), • 403(b) Retirement Plan, • Shift Differentials, • Flexible Spending Account, • Paid time off, vacation and sick time, • Tuition reimbursement, • Paid birthday Qualifications: • Must possess a current, unencumbered, active Registered Nurse license, • Must have at least 2 years of recent experience in the direct care of adults or chronically disabled persons, • Must be CPR and first aid certified, • Must be familiar and comply with the Nurse Practice Act, • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to post-acute care, • Must have one (1) year of experience in long-term care, home care or assisted living, • Must possess leadership ability and willingness to work harmoniously with and supervise other personnel, • Ability to plan, organize, develop, implement, and interpret programs, goals, objective, policies, procedures, etc. of the Nursing department and that are necessary for providing quality care, • Must have written and verbal communication skills, • Ability to move throughout the workday and perform physical activities, including sitting, standing, bending, twisting, stooping, lifting/moving/pushing/pulling up to 60 lbs. without assistance, • Ability to see, hear, feel, read, write, and verbalize clearly in English Duties and Responsibilities: • Provision and supervision of all nursing services and nursing staff, • Coordinating the completion of, and ensuring the accuracy of each participant's assessment, • Coordinating the development, ongoing review, and revision of each participant's plan of care, • Ensuring that nursing notes are written at least one (1) time per month or more often as necessary to reflect each participant's health care status, • Responding appropriately to any acute changes in a participant's health or functioning, • Providing appropriate health care education for each participant and legally authorized representative, • Assisting as necessary in the delivery of other services, • Ensuring ongoing education, training and professional development of all program staff that is relevant to care of participants, • Obtaining, reviewing and implementing primary care provider orders, • Coordinating the participant's care with primary care provider and, if applicable, community service providers, • Supervising medication management services JGS Lifecare conducts extensive background checks and professional references for all potential employees.
