The HopeHealth Hulitar Hospice Center, a 30-bed inpatient unit, is the only freestanding hospice center in Rhode Island. HopeHealth employs over 500 medical, clinical, operations and administrative staff, headquartered in Providence and with offices in Wakefield, RI and Attleboro, MA. Through its strong partnerships with regional health care organizations, HopeHealth serves more than 13,000 patients, and 2,000 children and adults in grief support programs. These services cover the entire state of Rhode Island—including Block Island—as well as 60 towns in southeastern Massachusetts. About the Position: The Community Services Coordinator assists with the coordination and administrative functions of all areas under clinical outreach and community services including grief support, volunteer services, community education, caregiver programs, support groups, PediPal and Alzheimer’s program. The position also plays an important role in supporting HopeHealth’s growth strategies by working in the community to further the awareness and understanding of services provided by HopeHealth. The role works in close collaboration within the direction of the Sr. Director of Clinical Outreach & Community Relations and managers within that discipline. This is both an internal and external facing role at HopeHealth. ESSENTIAL DUTIES AND RESPONSIBILITES • Coordinates and assigns volunteers to support group facilitation and community facing events that are consistent with the goals, definition, philosophy, and purpose of HopeHealth., • Creates and maintains a detailed and updated/real time calendar of all community events that HopeHealth participates in and will participate ensuring proper representation has been secured and appropriate event strategy is in place., • Provides first line customer service to programs within Community Services department by responding to customer inquiries. Provides information to those inquiring or routes inquiries to appropriate personnel, takes messages or relates information in a confidential, accurate, timely and professional manner., • Assists in the planning and implementation of year-round programming for all departments within community services including but not limited to Camp BraveHeart, Weekend of Hope & Healing, annual remembrance events, virtual and community education, volunteer appreciation and other events, Pedi Pal family events, and the Alzheimer’s Program., • Responsible for data management as it pertains to all community service programs and performs data entry in alignment with the needs of the community services department., • Collaborates with department managers, to support the creation and maintenance of all communication and support materials internally and externally for Community Services Department. Maintains all new collateral in Community Resource Database. • Coordinate services with other program staff members and external vendors. Register callers for various community service programs as needed. Register attendees, create invoices, collect credit card or check payments for community services programs. Submitting, tracking and following up with submissions of invoices, and reimbursement receipts. Assist with set up and registration for community service events., • Monitors internal folders and documents for all community services departments, updating as needed and notifying staff of changes., • Coordinate all tasks related to various ongoing mailings for community services department including bereavement monthly mailing, quarterly dementia caregiver mailing, and support group schedules. May coordinate work for office volunteers to assist with these tasks and others., • Provide outreach to and attend community fairs representing HopeHealth in areas of interest to HopeHealth as an expansion of community development., • Is a subject matter expert in HopeHealth services and is able to clearly articulate to audiences the value of services we provide., • As directed by manager, conducts community outreach representing HopeHealth services in alignment with growth strategies., • Consistently apply the HopeHealth values in all aspects of work, complying with established HopeHealth mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPAA compliance., • Special projects and other duties as requested by Senior Director of Clinical Outreach and Community Relations. QUALIFICATION REQUIREMENTS • Associate degree in Human Services, Communications, Public Relations, Marketing, Healthcare Administration, Social Services, or a related field; equivalent combination of education and relevant experience may be considered., • Minimum of 2 years of experience in community services, nonprofit programming, healthcare, social services, event coordination, volunteer coordination, or a related field., • Demonstrated ability to coordinate multiple programs, projects, events, and priorities simultaneously while maintaining attention to detail and follow-through., • Strong organizational and time-management skills, with the ability to maintain accurate records, schedules, program information, participation data, and follow-up activities., • Proficiency with Microsoft Office/365 and the ability to learn and effectively utilize databases, scheduling platforms, and other program-tracking tools., • Excellent verbal and written communication skills., • Ability to work independently while collaborating effectively with colleagues across departments., • Ability to represent HopeHealth professionally and authentically at community events, meetings, educational programs, and partner activities., • Strong interpersonal skills and demonstrated ability to build positive, respectful relationships with individuals and organizations throughout the community., • Ability to engage comfortably with diverse audiences and adapt communication style to different settings and populations., • Ability to manage changing priorities, competing deadlines, and frequent interruptions while maintaining professionalism and composure., • Commitment to the mission and values of HopeHealth and an understanding of the importance of hospice, palliative care, grief support, and community-based services.
