THE POSITION The Institutional Giving Specialist is a part-time, frontline fundraising position responsible for operating support from corporate and foundation sources, with government agencies as assigned. The Specialist will produce proposals, budgets, reports, and financials for institutional funders, including general operating support and program support in the visual and performing arts, horticulture, composting, urban farming, education, and workforce development. The successful candidate will have a proven track record of growing an institutional giving program, securing major grants, and engaging corporate sponsors. This position will report to the Executive Vice President (EVP), who oversees fundraising, board development, and government relations. This position will interface with the President & CEO, Senior Directors of the Regional Brand Experience, Community Impact, and Campus Administration teams, and program staff. The Specialist will be expected to bring strategy and robust implementation to Snug Harbor’s institutional giving, resulting in substantial revenue growth. While the role is available for remote work, some time onsite at Snug Harbor will be required. Responsibilities Oversee and manage institutional operating fund grants • Develop and implement an institutional funding plan to meet annual and long-term fundraising goals • Prepare and submit proposals to underwrite operating fund and programs to corporations and foundations, and government agencies as assigned • Track proposal deadlines, reporting, and financials, and grant compliance • Prepare fundraising reports, budgets, and analyses • Research, identify, and engage prospective funders • Stay informed about fundraising trends, techniques, and technology • Manage funder profiles in Patron Manager; with Development Assistant, enter gifts, generate acknowledgments Support Snug Harbor’s comprehensive fund development efforts • Develop a thorough understanding of Snug Harbor programs, initiatives, and needs • Collaborate with EVP, leadership, and program staff to develop annual and campaign institutional fundraising strategies • Support EVP and CEO with their institutional funder cultivation and stewardship • With EVP and Senior Director, Marketing & Brand Experience, collaborate on key messaging and communication strategies Qualifications • Minimum three years’ experience in institutional giving (corporate, foundation) • Demonstrated success in meeting/exceeding fundraising goals • Excellent oral, written, and interpersonal communication skills; fluency in English required • Strong project- and time-management skills • Ability to work independently and as part of a team • Facility with Patron Manager or similar CRM software; Microsoft Office suite • Availability to be on-site at Snug Harbor as required SALARY Salary is commensurate with experience, with a range of $35.00-$40.00 per hour. The position is part-time, approximately at 20 hours per week, or up to 85 hours per month on average. The role is available for remote work, although time onsite at Snug Harbor will be required. Help families find the help they need! JOB IS IN QUEENS - REMOTE AND ON THE FEILD Work with families and navigate the Medicaid system with them. Help them find housing, mental health services, occupational health services, and all other New York programs. This job is partly remote and partly on the field. You'll be going to families' homes and speaking with them about what services they may need, helping them find those services, and helping refer them over too! Job duties: - Complete initial and annual comprehensive assessment of medical, behavioral health, and social service needs for the assigned health home enrollees. - Provide disease-specific education and information regarding community resources. - Collaborate with a variety of community providers and resources to obtain needed services and support, utilizing community and family resources to create a sustainable support system. - Request and coordinate team and patient meetings as needed or requested by patient/family and/or team and/or escalate care management when medical assessment is needed. - Ensure that diagnostic, post-hospitalization, and specialty referrals have been executed and that results received and acted upon as needed. - Document plan of care, patient utilization, activities, and other required information with the State and EMR. - Monitor assigned enrollees' utilization of services, ensuring care is accessible, attended, and effective. - Provide regular data to the team on patient compliance and strategies to improve patient compliance. - Participate in on-call activities as directed/scheduled by the Program Coordinator. - Participate in regularly scheduled team meetings as prescribed by the practice's policy. - Participate in cultural competency events and training appropriate to job duties. - Frequent non-medical management coaching, education, follow-up visits, and phone calls to patients to monitor progress and identify new barriers or concerns. - Assisting with financial or other social issues that may provide barriers to patient compliance - Providing education/guidance to patients and family on tools to manage chronic illnesses, developing individual and web-based tools and resources to improve compliance. - Identifying and connecting patients with community resources to assist with improving compliance with treatment protocols and social issues (e.g. legal aid). - Accurately and timely document all interventions into prescribed electronic medical record systems to ensure timely reimbursement in compliance with New York State Health Home regulations and Patient-centered medical home regulations. - Participate in patient/outpatient care training regarding the care management strategies for difficult-to-manage patients, and educate office staff on patient or office system issues, including communicating patient care inconsistencies between the primary care physician and referring specialists. Job Qualifications - The Health Home Care Manager must have a BA and a minimum of 2 years of relevant experience in Human Care Services. - Excellent communication and team skills, including the ability to form strong collaborative interdisciplinary partnerships across care settings. - Sound computer knowledge and skills, including an aptitude for using health information technology to guide activities. - Ability to work independently and meet deadlines. As our Events + Public Space Programming Associate you will … The Events + Public Space Programming Associate plays a key role in planning and executing numerous public events in Union Square, specializing in on-site logistics and partner activations. Reporting to the Director of Marketing, Events, + Partnerships, your job will be to ensure seamless event operations and to cultivate strong relationships with external partners. You will be the on-the-ground leader, responsible for the smooth execution of events from start to finish. The Events + Public Space Programming Associate responsibilities include, but are not limited to: USP Stakeholder Events: ▪ Manage logistics for recurring stakeholder events, including USP’s Fall and Spring Board of Directors Meetings, USP's Annual Membership Meeting, and our Holiday Party. ▪ Plan and execute ad-hoc stakeholder events such as networking receptions, volunteer initiatives, community forums, and press conferences. ▪ Serve as the primary on-site coordinator for USP at all stakeholder and community events, ensuring seamless operations and alignment with organizational goals.
