Job Description We are seeking an experienced and detail-oriented Service Coordinator to oversee and manage homecare services for individuals in the Nursing Home Transition & Diversion (NHTD) program. The ideal candidate will have a strong understanding of NHTD procedures, policies, and compliance requirements in homecare settings. Responsibilities: • Coordinate and manage homecare services for NHTD clients, ensuring quality care and compliance., • Serve as the primary point of contact for clients, families, and caregivers., • Develop, implement, and monitor individualized service plans., • Ensure compliance with state regulations and program policies., • Communicate effectively with healthcare professionals, service providers, and agencies., • Maintain accurate documentation and reports on client care and service coordination., • Prior experience in NHTD, TBI waiver programs, or homecare service coordination., • Strong knowledge of NYS DOH regulations and Medicaid-funded programs., • Excellent organizational and communication skills. • • Ability to work independently while coordinating with multiple stakeholders.
