Housing Specialist The in-shelter Housing Specialist is responsible for supporting shelter residents in securing stable housing through comprehensive assessment, planning, and placement services. This role involves developing individualized housing plans, assisting clients in navigating NYC's housing voucher systems, coordinating housing searches, and maintaining strong partnerships with landlords and housing providers. The aim of Community First is to create an upstream diversion from arrest and justice system involvement by building supportive relationships with people in need of services and creating linkages to mental health services, housing support, substance use services, and other kinds of social services as needed. Community First is a partnership between Midtown Community Justice Center, Breaking Ground, Fountain House, Times Square Alliance, Housing Works, and Ryan Health. Reporting to the Clinical Director, the Community Navigator will be tasked with engaging community members frequenting the Times Square and Midtown Manhattan area who are experiencing housing insecurity or homelessness and often have co-occurring behavioral health challenges. In keeping with the Community First model, the Navigator will build trusting relationships with those individuals, and facilitate linkages to social services and other community-based resources. The Community Navigator will also work with clients of MCJC’s onsite programs, including court-based and diversion programs, who are unstably housed. Community Navigators are peers who share some lived experience with clients and use their own experience in establishing trusted relationships. The program serves individuals of all ages who are released at criminal court arraignment, who are charged with felonies and misdemeanors, and who present a substantial likelihood of detention but are at risk of failing to appear in court if released on recognizance. Participants engage with the program through on-going in person or phone check-ins and take part in a collaborative process to identify individual strengths, goals, and needs, such as health services, substance use support resources. Reporting to the Clinical Director of Felony ATI, the Counselor will be part of a team that provides neutral, third-party assessments and case management services to the Felony Alternative to Incarceration (ATI) Court. The Counselor will be responsible for conducting independent clinical assessments, providing referrals to community-based services, offering comprehensive clinical case management, and monitoring participant compliance. An essential and dynamic feature of the Counselor ’s role will be close collaboration with key stakeholders. The Felony ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to the Clinical Director of Felony ATI, the Social Worker will be part of a team that provides neutral, third-party assessments and case management services to the Felony Alternative to Incarceration (ATI) Court. The Social Worker will be responsible for conducting independent clinical assessments, providing referrals to community-based services, offering comprehensive clinical case management, and monitoring participant compliance. An essential and dynamic feature of the Social Worker’s role will be close collaboration with key stakeholders. The Social Worker will work regularly with the judge and court staff from New York Country Supreme Court who oversee the Felony ATI Court, as well as with other stakeholders, including the Manhattan District Attorney’s Office and New York City defense organizations. Court-based social work is an exciting, unique and demanding area of practice, and the Social Worker role involves navigating and constructively engaging with differing and, at times, competing stakeholder perspectives. Job Description Summary: Streetwork Project is the oldest street outreach program for homeless and street-involved youth in New York City. The program has grown from its beginnings in 1984 as a storefront project to encompass both outreach and comprehensive harm reduction services. The core service is long-term, one-on-one counseling, which is offered in a non-judgmental and holistic manner utilizing a harm reduction approach that focuses on building trust, fostering self-esteem, exploring options and choices, and empowering youth to change their high-risk behaviors. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • The role of the Housing Navigator is to assist households in receipt of a housing vouchers (primarily Section 8 and CityFHEPS, although occasionally other housing vouchers such as HASA) through the processes of locating, leasing, and moving into housing after their voucher expires. Job Description Position Summary: The Social Care Navigator I (SCN) is the front line of NYREACH’s Social Care Network, connecting Medicaid members to the social care services they need to thrive. The SCN conducts outreach, screens members for health-related social needs (HRSN) — including housing, food insecurity, transportation, and interpersonal safety — assesses eligibility for enhanced HRSN services, and provides hands-on navigation and closed-loop referrals to community-based resources. The SCN builds trusted relationships with members and follows each referral through to completion, ensuring members do not fall through the cracks between screening and service delivery. In addition to core screening and navigation responsibilities, this SCN position carries a special focus on nutritional health, supporting members in accessing healthy food resources and nutrition-related services.
