The Social Worker and Navigator will create connections to other City agencies and providers and act as “expediters” for resources such as health, mental health, employment training, housing, food, etc., to help young adult Participants (ages 18-21) with issues identified in their Individual Service Plans. Position: Case Management Navigator Reports To: Program Director Location: 392 Blake Avenue Brooklyn, NY 11212 What The Case Management Navigator Does: • Work with the Social Worker on the implementation of ISPs, • Provide outreach and continuous support to participants., • Collaborate with local Cure Violence providers and other neighborhood resources to create a comprehensive support network for participants and the program., • Identify high-risk participants in the community and conduct initial outreach to engage those not currently part of the program., • Maintain trust with identified participants and facilitate their connection with the Social Worker and Cornerstone activities., • Ensure meaningful connections with the community and local resources to support the program and neighborhood., • Meet with participants' families to understand their needs and provide necessary support and resources., • Act as a liaison between community and agency resources to enhance services for participants., • Identify, organize, and coordinate the activities of Peer Support Leaders., • Coordinate group development incentives and activities, including meals, recreation, trips, cultural events, and other opportunities for connection and growth., • Worked collaboratively with the Social Worker and under the direct supervision of the Van Dyke Cornerstone Director, • Assist in the collection of enrollment forms, documentation, and tracking of participant data to ensure compliance with program requirements., • Monitor clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face to face office visits and document via progress notes., • Recommend closing of cases in which clients have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area)., • Provide all required information for weekly/monthly/quarterly/annual reports., • As needed conduct client periodic re-assessments., • As needed schedule appointments for client with referral organizations., • As needed escort clients safely to and from school, • Other duties as assigned.Minimum Education/Experience Required:, • Bachelor's degree (B. PURPOSE OF POSITION The Patient Navigator provides support, advocacy, and care coordination for individuals accessing mental health and supportive counseling services, with a specialized focus on People Living with HIV/AIDS (PLWHA). The Patient Navigator works closely with clinical and support staff to ensure clients receive comprehensive care, including mental health services, substance use support, housing assistance, and linkage to medical care and social service providers. THE OPPORTUNITY The Center for Justice Innovation is seeking a Social Worker to join a team of staff—including social workers, case managers, and a peer navigator—who work with defendants participating in the Manhattan Felony Alternative to Incarceration (ATI) Court. The Felony ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to the Clinical Director of Felony ATI, the Social Worker will be part of a team that provides neutral, third-party assessments and case management services to the Felony Alternative to Incarceration (ATI) Court. The Social Worker will be responsible for conducting independent clinical assessments, providing referrals to community-based services, offering comprehensive clinical case management, and monitoring participant compliance. An essential and dynamic feature of the Social Worker’s role will be close collaboration with key stakeholders. The Social Worker will work regularly with the judge and court staff from New York Country Supreme Court who oversee the Felony ATI Court, as well as with other stakeholders, including the Manhattan District Attorney’s Office and New York City defense organizations. Court-based social work is an exciting, unique and demanding area of practice, and the Social Worker role involves navigating and constructively engaging with differing and, at times, competing stakeholder perspectives. THE OPPORTUNITY The Center for Justice Innovation is seeking a Counselor to join a team of staff—including social workers, case managers, and a peer navigator—who work with defendants participating in the Manhattan Felony Alternative to Incarceration (ATI) Court. The Felony ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to the Clinical Director of Felony ATI, the Counselor will be part of a team that provides neutral, third-party assessments and case management services to the Felony Alternative to Incarceration (ATI) Court. The Counselor will be responsible for conducting independent clinical assessments, providing referrals to community-based services, offering comprehensive clinical case management, and monitoring participant compliance. An essential and dynamic feature of the Counselor ’s role will be close collaboration with key stakeholders. The Counselor will work regularly with the judge and court staff from New York Country Supreme Court who oversee the Felony ATI Court, as well as with other stakeholders, including the Manhattan District Attorney’s Office and New York City defense organizations. Court-based work is an exciting, unique and demanding area of practice, and the Counselor role involves navigating and constructively engaging with differing and, at times, competing stakeholder perspectives. Job Placement Assistance/Coaching • Assisting clients with strategic planning for entry level and SUD sector employment opportunities., • Collaborating with government and private workforce entities., • Build and maintain relationships with employers looking to hire professionals in SUD industry., • Collaborate with government funded or private employment training systems to facilitate client access., • Aid in employer referral opportunities for clients, • Maintain ongoing employer relationships where appropriate., • Identify weekly sector specific job leads via employment search engines for referral to job seekers., • Schedule and conduct weekly one on one job search coaching sessions as directed by client’s career development plan Peer Training Institute Graduate Follow-up • Provide comprehensive graduate case management for a caseload of engaged participants which includes career coaching, professional portfolio development and employment referrals., • Update and refer graduates to active graduate resource list to support retention outcomes related to employment and recovery., • Act as point person for re engagement with displaced graduates or graduate job seekers, connecting them to in house housing support, temporary work contract opportunities and overall career development support., • Support retention outcomes and the collection of retention information through monthly outreach to graduates. Job Description Summary: Streetwork Project is the oldest street outreach program for homeless and street-involved youth in New York City. The program has grown from its beginnings in 1984 as a storefront project to encompass both outreach and comprehensive harm reduction services. The core service is long-term, one-on-one counseling, which is offered in a non-judgmental and holistic manner utilizing a harm reduction approach that focuses on building trust, fostering self-esteem, exploring options and choices, and empowering youth to change their high-risk behaviors. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • The role of the Housing Navigator is to assist households in receipt of a housing vouchers (primarily Section 8 and CityFHEPS, although occasionally other housing vouchers such as HASA) through the processes of locating, leasing, and moving into housing after their voucher expires. This is a predominately field based position and will require travel within the five boroughs and possibly to adjacent communities in NYS and NJ if a household is interested in porting., • Housing Navigators will coordinate with DYCD to manage an ongoing caseload of individuals/households in receipt of housing vouchers. Essential Job Functions: • Engaging with Streetwork Project clients, Streetwork Project staff, and DYCD to connect with households in receipt of housing vouchers., • Engaging in phone and field-based outreach to landlords, brokers, and realtors to find apartments our clients can apply for., • Assist the household with acquiring the necessary documentation and mitigate issues with credit reports, utility arrears, and unfavorable landlord references as necessary., • Assisting voucher recipients with the process of securing housing including but not limited to:, • Providing participants with listings of potential housing options in alignment with participants’ housing needs and geographic preferences, • Coordinating and attending apartment viewings, • Acquiring and submitting apartment applications, supporting documentation, and forms, • Navigating the various rental subsidy and inspection processes, • Securing applicable move-in assistance (including HRA payments and vouchers), • Coordinating move-ins with landlords and other relevant stakeholders, • Adding unit to HOME database, when necessary, • Explain and review the lease and sublease agreement with the voucher recipient, • Educating voucher recipients on their rights and responsibilities as tenants., • Advocating for clients who experience discrimination in their housing search, including working with external attorneys and the City Commission on Human Rights, • Maintain accurate, comprehensive and current documentation and provide reports as needed., • Follow up with tenants once they are placed housing, • Coordinate with the Senior Housing Coordinator, Director of Housing and Resource Benefits, and RHY Team at DYCD Secondary Job Functions: • Engagement of youth into program structure (harm reduction philosophy, rules, mutual respect), • Facilitate access to daily living needs (food, clothes, hygiene supplies, space to rest) as needed, • Perform related duties and participate in special projects as assigned. Interacts with: • All Streetwork Sites, working closely with the following: clients, Program Coordinator of Housing Navigation, Housing Navigator, Director of Housing & Benefits Resources, Case Managers, • External stakeholders, including DYCD; NYCHA; HRA; landlords and property management companies Qualifications: Preferred Knowledge and Skills: • Experience with the housing process and housing applications in NYC., • Experience working with landlords, brokers and the rental market in NYC., • Knowledge of applicable fair housing laws., • Experience in case management or project management., • Proficiency with computers navigating in multiple systems and web-based applications., • Strong oral and written communication skills., • Ability to prioritize responsibilities, handle multiple projects simultaneously and delegate responsibilities when appropriate, • Have strong attention to detail, • Have excellent interpersonal skills, • Experience working with youth and young adults experiencing homelessness.
