Job DescriptionDescription: Summary: Under the supervision of HCBS Program Supervisor the Supported Employment and Family Support Services Coordinator is responsible for coordinating, overseeing, and supporting employment services and family-focused support for individuals with intellectual and developmental disabilities. The coordinator ensures services are person-centered, compliant with OPWDD regulations, and responsive to the needs of individuals and families. Reasonable Accommodations may be made to enable qualified individuals with disabilities to perform the essential functions. • Coordinate and oversee Supported Employment services, including job development, job coaching, and employment retention supports., • Ensure employment services align with each individual’s Life Plan, employment goals, and Staff Action Plan., • Monitor direct and indirect SEMP services for quality, effectiveness, and compliance., • Maintain relationships with employers, community businesses, and employment partners., • Track employment outcomes, progress, and challenges; adjust support as needed., • Participate in Life Plan meetings and employment-related team meetings., • Supervise assigned staff, including job coaches., • Provide regular supervision, coaching, and performance feedback., • Ensure staff complete accurate documentation and data collection., • Support staff training and professional development., • Ensure timely and accurate documentation for all services provided., • Review progress notes, data collection, monthly summaries, and reports for compliance., • Maintain records in accordance with OPWDD, Medicaid, and agency requirements., • Support audits, reviews, and quality assurance activities., • Collaborate with care managers, OPWDD, ETP, community partners, and internal teams., • Identify opportunities to expand services, partnerships, and community engagement., • Support program growth, outreach, and enrollment., • Coordinate family support services, resources, and referrals in accordance with OPWDD Family Support Services guidelines., • Serve as a primary point of contact for families, providing information, guidance, and advocacy., • Support families in navigating OPWDD services, benefits, and community resources., • Plan and coordinate family workshops, support groups, and informational sessions., • Ensuring family support is culturally responsive and accessible., • Maintain regular communication with families and stakeholders. This position holds a relatively high degree of independence within the framework of the agency's policies and procedures. The individual must be resourceful and able to display initiative and understanding of management systems and interpersonal relations. Must keep immediate supervisor abreast of issues and concerns which could constitute intervention from higher level management. The position does require occasional standing, squatting, lifting of up to approximately 30 lbs. And frequent sitting.
